Patient End-of-Life Experiences for Pediatric Trainees: Spanning the Educational Continuum
Conclusions. Trainees experienced patient deaths along the educational continuum. These findings have implications for the optimal timing and method of end-of-life care education. (Source: Clinical Pediatrics)

Postpartum Depression in a General Pediatric Practice: Practical Methods for Improving Screening and Referrals
We examined the effects of an educational session about PPD and modification of the electronic medical record (EMR) on providers’ screening for PPD. An education session was given to the physicians and pre and post surveys compared comfort and self-reported screening. Following the EMR change, a retrospective chart review was conducted from three time periods: group 1—before the conference, group 2—after the conference but before EMR change, and group 3—after screening in the EMR. Documented screening increased from 0% in group 1, to 2% in group 2, to 74% in group 3 (P < .001). Ten percent screen...

Late ELISA Testing in Infants Born to HIV-Positive Mothers
We present the case of a young boy who was born to a human immunodeficiency virus (HIV)-positive mother and originally found to be uninfected. Evidence-based guidelines were followed regarding the mother’s prenatal and infant’s postnatal care, including the avoidance of breast milk. HIV DNA polymerase chain reaction qualitative tests were obtained at birth, 6 weeks and 4 months, and were all negative. He also received 6 weeks of prophylactic zidovudine. Despite these measures, his health began to decline at 17 months of age and antibody and serology tests performed at this time confirmed HIV infection. Guidelin...

Hypernatremia in Children With Diarrhea: Presenting Features, Management, Outcome, and Risk Factors for Death
We sought to investigate the magnitude, clinical features, treatment, and outcome of children suffering from hypernatremic diarrhea and to identify risk factors for fatal outcome among them. We reviewed 2 data sets of children <15 years admitted to the in-patient ward of the Dhaka Hospital of International Centre for Diarrhoeal Disease Research, Bangladesh (icddr, b) with diarrhea and hypernatremia (serum sodium ≥150 mmol/L): (a) March 2001 to March 2002 (n = 371) and (b) March 2009 to August 2011 (n = 360). We reviewed their records and collected relevant information for analyses. The prevalence of hypernatremia was...

Utility of Obtaining a Serum Basic Metabolic Panel in the Setting of a First-Time Nonfebrile Seizure
New-onset nonfebrile seizures in an otherwise healthy child are common, affecting 25 000 to 40 000 U.S. children annually. We hypothesized seizure-provoking electrolyte disturbances such as hyponatremia, hypoglycemia, and hypocalcemia are uncommon in these children. From January 1, 2009 to May 31, 2009, 358 children aged 29 days to 18 years with a diagnosis code of 780.39 ("other convulsions" including "first time seizure," etc) were included for potential retrospective review. Children with known epilepsy and febrile seizures were excluded. Electrolytes were obtained in nearly all children with a history suggestive of an ...

Hazard Patterns Associated With Co-sleepers
We conducted a retrospective review of incidents associated with co-sleepers. A total of 26 incidents (6 deaths and 20 injuries) were reported to the Consumer Product Safety Commission. Of these, 5 deaths were caused by asphyxia, and 1 was attributed to sudden infant death syndrome (SIDS). The mean age at death was 3.1 months. Two-thirds of deaths had known risk factors for SIDS. Of the 20 injuries, entrapment (60%) and suffocation (35%) hazards were most common. Almost half of the injuries occurred after the co-sleeper was improperly assembled. Infant deaths and injuries associated with co-sleepers are infrequent. Most de...

The Relationship Between Gastroesophageal Reflux and Chronic Unexplained Cough in Children
The aim of this study was to assess the association between chronic cough and acid or weakly acid gastroesophageal reflux (GER) determined by 24-hour multichannel intraluminal impedance-pH monitoring and to assess whether the association is age dependent. Overall 150 children (mean age 7.5 years; range 0.3-18.0 years; male/female 90/60) were enrolled. Median of 87.5% (0% to 100%) of all cough episodes were associated with reflux; 9% (0% to 100%) with acidic and 60% (0% to 100%) with weakly acidic episodes. In 52 children (34.7%), all cough episodes were associated with GER (100% association). Children younger than 2 years ...
