High Risk of Surgical Glove Perforation From Surgical Rotatory Instruments.
Abstract BACKGROUND: Surgical gloves can be damaged during the course of a procedure, which can place the surgeon and patient at risk. Glove perforation may not always be readily apparent, and determining the risk factors for glove perforation can aid the surgeon in deciding when a glove change is advisable. Time of wear and needle sticks have been well studied; however, other mechanisms including mechanical stress from surgical equipment have had limited evaluation to date. QUESTIONS/PURPOSES: We evaluated the risk of glove perforation in gloves that were caught in a surgical rotatory device (such as drills ...

Does Surface Topography Play a Role in Taper Damage in Head-neck Modular Junctions?
CONCLUSIONS: Stem taper surface topography was related to damage scores on retrieved head-neck modular junctions; however, it affected CoCr/CoCr and CoCr/Ti couples differently. CLINICAL RELEVANCE: A taper topography of circumferential machining marks with higher peaks appears to enable slower damage progression and, subsequently, a reduction of the reported release of corrosion products. This may be of interest to implant designers and manufacturers in an effort to reduce the effects of metal release from modular femoral components. No Difference in Early Analgesia Between Liposomal Bupivacaine Injection and Intrathecal Morphine After TKA.
Abstract BACKGROUND: Opioid analgesics have been a standard modality for postoperative pain management after total knee arthroplasty (TKA) but are also associated with increased risk of nausea, pruritus, vomiting, respiratory depression, prolonged ileus, and cognitive dysfunction. There is still a need for a method of anesthesia that can deliver effective long-term postoperative pain relief without incurring the high cost and health burden of opioids and nerve blocks. QUESTIONS/PURPOSES: (1) Is liposomal bupivacaine-based periarticular injection (PAI) more effective than morphine-based spinal anesthesia or ro...

CONCLUSIONS: The accuracy of model-based RSA for measurement of the y axis rotation was not quite as high as that of marker-based RSA, but its clinical precision is at an acceptable level. Cement Augmentation in Sacroiliac Screw Fixation Offers Modest Biomechanical Advantages in a Cadaver Model.
CONCLUSIONS: The addition of cement to standard sacroiliac screw fixation seemed to change the mode and dynamics of failure in this cadaveric mechanical model. Although no advantages to cement were observed in terms of screw motion or cycles to failure among the different constructs, a cemented, two-screw sacroiliac screw construct resulted in less screw subsidence and greater energy absorbed to failure than an uncemented single transsacral screw. CLINICAL RELEVANCE: In osteoporotic bone, the addition of cement to sacroiliac screw fixation might improve screw anchorage. However, larger mechanical studies usi...

The Role of Highly Selective Implant Retention in the Infected Hip Arthroplasty.
CONCLUSIONS: In some patients, staged revision of large and well-fixed components will result in bone damage and compromised function. These results suggest that partial implant retention and joint débridement may be an alternative for those patients who have complex well-fixed acetabular or femoral components, are not immunocompromised, have not developed sinus formation, and we were able to obtain a positive hip aspirate. We caution this technique should not be applied when patients have chronic illness such as diabetes or rheumatoid disease, have a negative hip aspirate for microorganisms, or show any signs of lo...

A Novel Technique for Assessing Antioxidant Concentration in Retrieved UHMWPE.
Abstract BACKGROUND: Antioxidants added to UHMWPE to prevent in vivo oxidation are important to the long-term performance of hip and knee arthroplasty. Diffused vitamin E antioxidant polyethylene raised questions about potential in vivo elution that could cause inflammatory reactions in periprosthetic tissues and also potentially leave the implant once again prone to oxidation. Currently, there is no information on the elution, if any, of antioxidants from implant polyethylene materials in vivo. QUESTIONS/PURPOSES: (1) Do antioxidants, especially diffused vitamin E, elute from antioxidant polyethylene in vivo...

No Difference Between Trabecular Metal Cones and Femoral Head Allografts in Revision TKA: Minimum 5-year Followup.
CONCLUSIONS: With the numbers available, we found no difference in pain, function, or repeat revision when comparing femoral head allografts and trabecular metal cones for severe bone defects during revision TKA. However, we used allografts for the larger bone defects. Based on these results, we believe that femoral head allografts and trabecular metal cones can both be used for the management of Anderson Orthopaedic Research Institute Types 2 and 3 defects. Future multicenter studies are required with larger numbers, cost analyses, and a longer duration of followup. LEVEL OF EVIDENCE: Level III, therapeutic study. ...

Hip, Knee, and Ankle Osteoarthritis Negatively Affects Mechanical Energy Exchange.
Abstract BACKGROUND: Individuals with osteoarthritis (OA) of the lower limb find normal locomotion tiring compared with individuals without OA, possibly because OA of any lower limb joint changes limb mechanics and may disrupt transfer of potential and kinetic energy of the center of mass during walking, resulting in increased locomotor costs. Although recovery has been explored in asymptomatic individuals and in some patient populations, the effect of changes in these gait parameters on center of mass movements and mechanical work in patients with OA in specific joints has not been well examined. The results can ...

Hips With Protrusio Acetabuli Are at Increased Risk for Failure After Femoroacetabular Impingement Surgery: A 10-year Followup.
t M Abstract BACKGROUND: Protrusio acetabuli is a rare anatomic pattern of the hip in which the femoral head protrudes into the true pelvis. The increased depth of the hip and the excessive size of the lunate surface typically lead to severe pincer-type femoroacetabular impingement (FAI); however, to our knowledge, there are no published mid- or long-term studies on results of circumferential acetabular rim trimming through a surgical hip dislocation for patients with this condition. The objective of this study was to use a national arthroplasty registry to assess whether the choice of bearings-metal-on-polyethylene (MoP), ceramic-on-polyethylene (CoP), ceramic-on-ceramic (CoC), or metal-on-metal (MoM)-is associated with differences in the risk of revision for deep infection, either (1) within 6 months or (2) over the entire period of observation, which spanned 15 years. METHODS: Data from primary THAs were extracted from the New Zealand Joint Registry over a 15-year period. A total of 97,889 hips were available for analysis.
