Patient Training and Patient Safety in Home Hemodialysis
Clin J Am Soc Nephrol. 2024 Jan 8. doi: 10.2215/CJN.0000000000000416. Online ahead of print.ABSTRACTThe success of a home hemodialysis (HD) program depends largely on a patient safety framework and the risk tolerance of a home dialysis program. Dialysis treatments require operators to perform dozens of steps repeatedly and reliably in a complex procedure. For home HD, those operators are patients themselves or their care partners, so attention to safety and risk mitigation is front of mind. While newer, smaller and more user-friendly dialysis machines designed explicitly for home use are slowly entering the marketplace, te...

Impact of Remote and Virtual Technology on Home Dialysis
Clin J Am Soc Nephrol. 2024 Jan 8. doi: 10.2215/CJN.0000000000000405. Online ahead of print.ABSTRACTIn the U.S., regulatory changes dictate telehealth activities. Telehealth was available to patients on home dialysis as early as 2019, allowing patients to opt for telehealth with home as the originating site and without geographic restriction. In 2020, COVID-19 was an unexpected accelerant for telehealth use in the U.S. Within nephrology, remote patient monitoring has most often been applied to the care of patients on home dialysis modalities. The impact that remote and virtual technologies have on home dialysis patients, t...

Peritoneal Dialysis Care for People with Diabetes, Polycystic Kidney Disease, or Advanced Liver Disease
Clin J Am Soc Nephrol. 2024 Jan 8. doi: 10.2215/CJN.0000000000000420. Online ahead of print.ABSTRACTPeople treated with peritoneal dialysis (PD) often have complicating conditions that require careful management. Three such conditions are reviewed in this article - diabetes mellitus, polycystic kidney disease (PKD) and chronic liver disease. Each of these conditions requires an understanding of both its impact on the delivery of the PD, as well as the impact of the PD on the condition itself. With respect to diabetes, glucose absorption from the dialysate complicates metabolic control, impacts on salt and water management ...

From Home to Wearable Hemodialysis: Barriers, Progress, and Opportunities
Clin J Am Soc Nephrol. 2024 Jan 8. doi: 10.2215/CJN.0000000000000424. Online ahead of print.ABSTRACTAlthough the past two decades have seen substantial proportional growth of home hemodialysis (HD) in the United States, the absolute number of patients treated with home HD remains small. Currently available stationary hemodialysis devices for use in the home have inherent limitations that are represent barriers for more widespread adoption by a larger proportion of individuals with kidney failure. These limitations include device weight and bulk, ergonomics considerations, technical complexity, vascular access challenges, a...

Evaluating ApoL1 Genetic Testing Policy Options for Transplant Centers: A Delphi Consensus Panel Project with Stakeholders
CONCLUSIONS: The panel generally supported policy options involving discussion and shared decision-making among patients, donors, and family stakeholders. There was general opposition to unilateral decision-making and prohibiting donation altogether.PMID:38190141 | DOI:10.2215/CJN.0000000000000397 (Source: Clinical Journal of the American Society of Nephrology : CJASN)

Complications of Peritoneal Dialysis Part II: Nonmechanical Complications
Clin J Am Soc Nephrol. 2024 Jan 8. doi: 10.2215/CJN.0000000000000418. Online ahead of print.ABSTRACTPeritoneal dialysis (PD) is a form of kidney replacement therapy that offers flexibility and autonomy to patients with end stage kidney disease. It is associated with lower costs compared to hemodialysis in many countries. Unlike mechanical complications that typical arise early in the course of treatment, non-infectious, non-mechanical complications often present late in patients that are established on PD. In this review, we first discuss abnormal-appearing drained dialysate, including hemoperitoneum, chyloperitoneum and n...

Protected Time for Program Administration among Nephrology Program Leadership in the United States
CONCLUSIONS: Program leaders estimated minimum necessary program administration times (based on program size) that exceeded ACGME requirements. Associate program directors/core faculty contributed substantially to non-clinical training. 34% of associate program directors and 25% of program directors had a high likelihood of burnout.PMID:38190147 | DOI:10.2215/CJN.0000000000000412 (Source: Clinical Journal of the American Society of Nephrology : CJASN)

The Association of Intra-abdominal Adhesions with Peritoneal Dialysis Catheter-Related Complications
CONCLUSIONS: People with intra-abdominal adhesions undergoing PD catheter insertion were at higher risk for abdominal pain or flow restriction preventing PD from starting, PD termination or requiring an invasive procedure. However, the majority of patients, with or without adhesions, did not experience complications, and most complications did not lead to the termination of PD therapy.PMID:38190176 | DOI:10.2215/CJN.0000000000000404 (Source: Clinical Journal of the American Society of Nephrology : CJASN)

The Burden of Home Dialysis: An Overlooked Challenge
Clin J Am Soc Nephrol. 2024 Jan 8. doi: 10.2215/CJN.0000000000000413. Online ahead of print.ABSTRACTHome dialysis offers several clinical and quality of life benefits for patients with kidney failure. However, it is important to recognize that home dialysis may place an increased burden on patients and their care partners. Sources of burden may include concerns about the ability to adequately and safely perform dialysis at home, physical symptoms, impairment of life participation, psychosocial challenges, and care partner burnout. Overlooking or failing to address these issues may lead to adverse events that negatively imp...

Complications of Peritoneal Dialysis Part I: Mechanical Complications
Clin J Am Soc Nephrol. 2024 Jan 8. doi: 10.2215/CJN.0000000000000417. Online ahead of print.ABSTRACTPeritoneal dialysis (PD) is a form of kidney replacement therapy that offers flexibility and autonomy to patients with end stage kidney disease. It is associated with lower costs compared to hemodialysis in many countries. However, it can be associated with unexpected interruptions to, or discontinuation of therapy. Timely diagnosis and resolution are required to minimize preventable modality change to hemodialysis. This review covers mechanical complications, including leaks, PD hydrothorax, hernias, dialysate flow problems...

Drug-Coated Balloon Angioplasty for Dysfunctional Arteriovenous Hemodialysis Fistulae: A Randomized Controlled Trial
CONCLUSIONS: AcoArt Orchid DCB showed better primary patency rates compared with plain balloon angioplasty for treating stenotic lesions in dysfunctional hemodialysis AV fistulae at 6 and 12 months. It required fewer repeated interventions and had comparable safety in 1 year.CLINICAL TRIAL REGISTRY NAME AND REGISTRATION NUMBER: AcoArt III/Arterio-venous Fistula in China, NCT03366727.PMID:38191561 | DOI:10.2215/CJN.0000000000000359 (Source: Clinical Journal of the American Society of Nephrology : CJASN)
