Dialysate Sodium Lowering in Maintenance Hemodialysis A Randomized Clinical Trial
CONCLUSION: Use of a dialysate sodium of 135 as compared with 138 mEq/L was associated with a small reduction in interdialytic weight gain without impact on IDH or predialysis BP, but with an increase in symptoms. Raising dialysate sodium from 135 to 140mEq/L was associated with a reduction in IDH, a small increase in interdialytic weight gain and a marked increase in predialysis BP.PMID:38349776 | DOI:10.2215/CJN.0000000000000431 (Source: Clinical Journal of the American Society of Nephrology : CJASN)

Preventive Pharmacological Therapy and Risk of Recurrent Urinary Stone Disease
CONCLUSIONS: When considering all urinary abnormalities together, this study demonstrates that use of preventive pharmacological therapy is associated with a lower risk of clinically significant recurrent episodes of urinary stone disease in the 12-36 month timeframe following urine collection, though only the association with use of alkali therapy for hypocitraturia was significant when individual abnormalities were examined.PMID:38345854 | DOI:10.2215/CJN.0000000000000428 (Source: Clinical Journal of the American Society of Nephrology : CJASN)

Should Transplant Nephrology pursue recognition from the Accreditation Council for Graduate Medical Education (ACGME)?
Clin J Am Soc Nephrol. 2024 Feb 6. doi: 10.2215/CJN.0000000000000441. Online ahead of print.ABSTRACTKidney transplant is not only the best treatment for patients with advanced kidney disease, but it also reduces health-care expenditure. The management of transplant patients is complex as they require special care by transplant nephrologists who have expertise in assessing transplant candidates, understand immunology and organ rejection, have familiarity with peri-operative complications, and have the ability to manage the long-term effects of chronic immunosuppression. This skill set at the intersection of multiple discipl...

Pregnancy in Patients Receiving Home Dialysis
Clin J Am Soc Nephrol. 2024 Jan 29. doi: 10.2215/CJN.0000000000000437. Online ahead of print.ABSTRACTPregnancy is an important goal for many women with chronic kidney disease (CKD) or kidney failure, but important barriers exist, particularly as CKD stage progresses. Women with advanced CKD often have a limited fertility window and may miss their opportunity for a pregnancy if advised to defer until after kidney transplantation. Pregnancy rates in women with advanced kidney failure or receiving dialysis remain low, and despite the improved outcomes in recent years, these pregnancies remain high risk for both mother and bab...
