Sudan black poisoning resulted in methemoglobinemia in a baby with congenital chyloperitoneum
Key Clinical MessageTreatment of congenital chyloperitoneum is a challenge. Conservative methods may be ineffective. Preoperative visualization of the site of lymphatic leakage is crucial, but radiological imaging is technically complicated and may not provide sufficient information, especially in small patients. To ease the detection of lymphatic leakage during surgery, preoperative feeding with fat-rich formula with Sudan Black has been recommended. However, administration of Sudan Black may result in life-threatening methemoglobinemia and liver damage without any advantage of revealing leakage during surgery. We recomme...

Consanguineous couple with SDHD gene mutations: Diagnosis, treatment, and implications of family genetic testing
DOTATOC-PET/CT of both spouses; the upper one showing no evidence of a cervico-thoraco-abdominal lesion suspicious for malignancy (wife), the lower one showing a stable glomus tumor on the right side of the neck (husband). Key Clinical MessagesThe newly published clinical consensus guideline on the management of PGL/PCC is helpful for decision-making for diagnostics and treatment. Still, the treatment of patients with SDHD gene mutations requires an individual approach and those patients belong to multiprofessional teams. It is often assumed that spouses are genetically unrelated. However, the genetic relationships between...

Iron deficiency ‐induced thrombocytosis presenting with blue toe syndrome
We report the case of a 38-year-old Saudi male who presented with blue toe syndrome and laboratory investigations revealed severe thrombocytosis secondary to iron deficiency. The patient was managed with analgesics, antiplatelets, anticoagulation, intravenous fluids, and iron supplementation. Subsequently, his symptoms resolved within a few days. IDIT is crucial to consider as a possible cause of microvascular thrombosis disorders, especially in patients with severe thrombocytosis. (Source: Clinical Case Reports)

Case report: Emerging BRCA mutation confers benefit from olaparib after chemotherapy intolerance in advanced triple ‐negative breast cancer
Key Clinical MessageIn a patient with metastatic breast cancer, an acquired BRCA mutation in the BRCA gene was detected, resulting in benefits from olaparib treatment. This underscores the importance of ongoing genetic phenotype testing after paclitaxel chemotherapy.AbstractTriple-negative breast cancer (TNBC) is associated with a poor prognosis and elevated mortality risk. BRCA mutations are commonly regarded as prevalent mutations in TNBC patients, strongly associated with congenital familial heredity. Dynamic changes in mutation sites, however, are rarely reported. In this case report, we report a 59-year-old TNBC patie...

Antimicrobial ‐associated encephalopathy due to ampicillin
Key Clinical MessageBecause the β-lactam ring has a molecular structure similar to that of gamma-aminobutyric acid (GABA) neurotransmitters, it binds to GABA A receptors and inhibits GABAergic transmission, causing AAE. The possibility of antimicrobial-associated encephalopathy should be considered in cases of neurological or psy chiatric symptoms after initiating an antimicrobial regimen.
