Key Clinical MessageNocardiosis is a rare opportunistic infection that is classically observed in immunocompromised patients but can also affect immunocompetent individuals. It tends to involve the lung, central nervous system, and skin and is often misdiagnosed. (Source: Clinical Case Reports)

Multiple cutaneous myxofibrosarcoma on the right arm: Clinicopathological features and differential diagnosis
Multiple cutaneous myxofibrosarcoma on the right arm. Key Clinical MessageThe clinical presentations and pathological features of low-grade myxofibrosarcoma can be misleading, frequently resulting in diagnostic errors. An accurate diagnosis requires the application of immunohistochemistry techniques and the discerning diagnostic acumen of experienced pathologists. A 62-year-old male patient visited our outpatient clinic with multiple painful and rapidly enlarging subcutaneous nodules on his right forearm. Initially, the condition was misdiagnosed as multiple lipomas. The final pathology revealed characteristics consistent ...

Association of COVID 19 pneumonitis and acute mesenteric ischemia
Key Clinical MessageD. dimer could be useful as an indicator in diagnosis of mesenteric ischemia in COVID patients. A two staged damage control emergency laparotomy is of good benefits in such patients.AbstractBowel ischemia in COVID 19 patients is extremely rare condition results from migrating thrombus formed by a hypercoagulable inflammatory state that is frequently associated COVID 19 infection. A two staged damage control emergency laparotomy is of good benefits in general especially in those with active COVID 19 infection. (Source: Clinical Case Reports)

Congenital lobar emphysema: A diagnostic dilemma with coexistent congenital heart defects
Key Clinical MessageClinicians should think beyond pneumonia and left-to-right shunts when young children have persistent respiratory distress. Congenital lung anomalies, including congenital lobar emphysema, should be considered differential diagnoses. Chest X-ray and CT imaging should be conducted to gain insight regarding and establish diagnosis, respectively.AbstractCongenital lobar emphysema (CLE) is a rare and life-threatening congenital lung anomaly that often poses a diagnostic dilemma. The issue can be further confused with coexistent congenital heart defects (CHDs) with left-to-right shunts. The clinical presenta...

Forequarter amputation for open scapulothoracic dissociation: A case report and surgical technique
Communited fragments of the scapula removed in piecemeal fashion during forequarter amputation for open SD. Key Clinical MessageScapulothoracic dissociation is a rare upper extremity injury caused by high-velocity trauma over anterolateral shoulder leading to disruption of scapulothoracic articulation. It is considered as a limb threatening injury. Very few cases of scapulothoracic dissociation have been reported in medical literature. In this article, we report a unique case of open injury involving laterally displaced and comminuted scapula accompanied by redundant acromioclavicular joint, total brachial plexus injury, a...

Endodontic treatment of a C ‐shaped mandibular second molar with narrow dentinal thickness: A case report
This report presents endodontic treatment of a C-shaped mandibular second molar diagnosed with pulpal necrosis and asymptomatic apical periodontitis, that initial radiographic assessment revealed extremely thin (0.3  mm) width of the lingual wall of the canals prone to perforation. Mechanical preparation was performed through the anti-curvature technique and basically by chemical irrigation rather than mechanical instrumentation. The C-shaped isthmus was prepared up to 25/02 rotary system without dentinal def ect while maintaining adequate dentin thickness so as to not significantly reduce the strength of the root. Follo...

Descending aorta to right atrial fistula: Transcatheter embolization of a very rare anomaly with coils
We present a case report of a trisomy 21 infant with atrial and ventricular septal defects and small patent ductus arteriosus (PDA) complicated by the presence of descending aorta to right atrial (RA) fistula with large left to right shunt leading to rapid increase in pulmonary vascular resistance. Transcatheter occlusion of the fistula followed by closure of the PDA with Nit-Occlud coil systems led to decreased pulmonary pressure and resistance permitting successful surgical repair of the patient's intracardiac defects with good outcome over 3  years of follow-up. Descending aorta to RA fistula is a rare and distinct cl...

Recurrent peri ‐myocarditis: A rarer but unfortunate redundant presentation of thyroid storm
Key Clinical MessageThyroid storm represents a critical and life-threatening complication from hyperthyroidism, with a notable mortality risk. Limited literature reports have explored the correlation between thyroid storm and peri-myocarditis, although the precise pathophysiological underpinnings remain unclear. The pathophysiology of how thyroid storm and peri-myocarditis are associated is not clearly understood; however, unfavorable prognostic factors include atrial fibrillation and recurrent thyrotoxicosis. Here, we present a case concerning recurrent peri-myocarditis concomitant with a thyroid storm. (Source: Clinical Case Reports)

Trifoliate gallbladder: A rare congenital anomaly masquerading as acute cholecystitis
Key Clinical MessageTriple gallbladder, an extremely rare congenital anomaly, can mimic more common biliary conditions. Accurate diagnosis through imaging and a multidisciplinary approach is essential for timely surgical management, preventing complications, and ensuring better patient outcomes.AbstractThe multiplication of the gallbladder is a congenital malformation with a rare incidence of 1 in 4000 patients, among which the trifoliate gallbladder, even rarer, is included. Gallbladder anomalies are typically discovered incidentally while investigating conditions like gallstones, sedimentation, gallbladder inflammation, ...
