Gastric sarcoidosis diagnosed with endoscopic ultrasound
We report the case of a 46-year-old African American female who experienced chronic epigastric abdominal pain, recurrent nausea, vomiting, and diarrhea for 15  years. Despite extensive investigations, including multiple biopsies, she was misdiagnosed with cyclic vomiting syndrome. Subsequently, an endoscopic ultrasound (EUS) revealed prominent lymph nodes and gastric granulomas, leading to a diagnosis of GS. This case underscores the importance of consi dering sarcoidosis in patients with refractory abdominal symptoms and highlights the utility of EUS in diagnosing this rare condition. Severe dengue infection unmasking drug ‐induced liver injury: Successful management with N‐acetylcysteine
Key Clinical MessageClinicians in tuberculosis and dengue endemic regions should have heightened vigilance for drug-induced liver injury (DILI) overlapping with active infections, enabling prompt recognition and life-saving conservative management.AbstractSevere dengue and drug-induced liver injury (DILI) are significant independent risk factors for acute liver failure. The co-occurrence of these conditions significantly complicates clinical management. Here, we describe the case of a 21-year-old Nepali female who developed acute liver failure during antitubercular therapy (ATT). The patient, presenting with fever and naus...

Initial spinal epidural hematoma symptoms mimic artery dissection
Axial contrast-enhanced computed tomography of the thoracic spine reveals a spinal epidural hematoma at six vertebral levels (red arrows) Key Clinical MessageAn elderly patient with upper back pain and hypertension was diagnosed as having spontaneous spinal epidural hematoma (SSEH) after excluding artery dissection. The initial symptoms of SSEH mimic those of artery dissection, and the symptoms of spinal damage frequently appear later. Physicians should, therefore, be mindful of SSEH. Transhepatic approach of patent foramen ovale closure in the setting of congenital inferior vena cava anomaly
Key Clinical MessagePatent foramen ovale (PFO) is the most common interatrial septal abnormality. The indications for PFO device closure are still being evaluated, with the most common reason being to prevent cerebrovascular accidents (CVA) caused by paradoxical embolism of deep vein thrombosis (DVT) in the lower extremities. This procedure is usually performed through percutaneous intervention using femoral vein access. Here, we present a case of PFO closure using a transhepatic approach, as femoral vein access was not feasible due to an interrupted inferior vena cava (IVC). The patient had a prominent left-sided IVC, lar...
