Peri ‐procedural management of anticoagulation for atrial fibrillation catheter ablation in direct oral anticoagulant‐treated patients
ConclusionsOur data showed that uninterrupted DOAC therapy resulted in unpredictable and highly variable initial level of anticoagulation before catheter ablation. Moreover, they highlighted that even with DOAC interruption preventing from interference between DOAC, UFH and ACT, intra‐procedural UFH monitoring was complex. Altogether, our exploratory results call into question the appropriateness of transposing UFH dose protocols, as well as the relevance of ACT monitoring in uninterrupted DOAC‐treated patients. Clinical Predictors and Outcomes of Patients with Pericardial Effusion in Chronic Kidney Disease
SUMMARY BackgroundPericardial effusion is common in hospitalized patients with chronic kidney disease (CKD). We sought to identify predictors of pericardial effusion in CKD patients and to evaluate the impact of pericardial effusion on their mortality and morbidity. MethodsIn a retrospective nested case control study design, we analyzed hospitalized adult patients with CKD stage 4, 5, and end‐stage renal disease (ESRD) diagnosed with pericardial effusion. Randomly selected patients with CKD stage 4, 5, and ESRD without pericardial effusion were used as controls. ResultsWe analyzed 84 cases and 61 controls, of whom 44% an...

Posttraumatic stress disorder and mortality in VA patients with implantable cardioverter defibrillator
ConclusionIn this retrospective cohort study among more than 25,000 veterans undergoing ICD implantation almost 13% had a diagnosis of PTSD. Subjects with PTSD were significantly younger yet they had a higher incidence of CHD, major cardiac comorbidities, cancer, and mental health conditions. Our study did not find an association between presence of PTSD before or after ICD implant and incident death when adjusted for all covariates. (Source: Clinical Cardiology)

PCSK9 inhibitor valuation: A science ‐based review of the two recent models
Low‐density lipoprotein cholesterol (LDL‐C) has been extensively evaluated. Prospective cohort studies, randomized controlled trials, biology, pathophysiology, genetics, and Mendelian randomization studies, have clearly taught us that LDL‐C causes atherosclerotic cardiovascular disease. The newest class of drugs to lower LDL‐C, the proprotein convertase subtilisin/kexin type 9 (PCSK9) monoclonal antibodies, have been found to safely reduce LDL‐C approximately 60% when added to high‐intensity statin therapy. Because their cost is much greater than that of the currently available agents, their value has been ques...

Spontaneous coronary artery dissection in women: What is known and what is yet to be understood
Spontaneous coronary artery dissection is a nonatherosclerotic etiology of acute coronary syndrome, including sudden cardiac death, which frequently affects younger women. This review highlights contemporary knowledge regarding spontaneous coronary artery dissection demographics, prevalence, diagnosis, presentation, and associated conditions and risks, inpatient treatment, major adverse clinical events, and outpatient management decisions.
