Effect of ischemic compression on myofascial pain syndrome: a systematic review and meta-analysis
ConclusionIschemic compression, as a conservative and noninvasive therapy, only enhanced tolerance to pain in MPS subjects compared with inactive control. Furthermore, there was no evidence of benefit for self-reported pain. The number of currently included subjects was relatively small, so the conclusion may be changed by future studies. Big scale RCTs with more subjects will be critical in future. (Source: Chiropractic and Manual Therapies)

“What you feel under your hands”: exploring professionals’ perspective of somatic dysfunction in osteopathic clinical practice—a qualitative study
ConclusionsA panel of expert osteopaths consider the concept of SD as a valuable tool integrated into the osteopathic evaluation and treatment process. The shared concept and clinical application of SD is informed by person-centered care concepts and from the fields of neuroscience, cognitive and complexity science. Our study reports a common need among osteopaths to develop an evidence-based framework of SD to allow the best development of the osteopathic profession. (Source: Chiropractic and Manual Therapies)

Who are the chiropractic students favouring a limitless scope of practice? Exploring the relationship with personality, magical thinking, and academic achievement
ConclusionNot surprisingly, ChiroCon is closely related to LLSoP and both were linked to magical thinking. Therefore, the questionnaire ‘Magical Health Beliefs’ could be a useful instrument to screen future chiropractic students to prevent a mismatch between student and institution. Depending on the outlook of the school, some schools would welcome these students, whereas other institutions would want to avoid them in their educ ation program. Management of people with low back pain: a survey of opinions and beliefs of Dutch and Belgian chiropractors
ConclusionsChiropractic adjustments were the most common self-reported treatment modalities supplemented by exercise in the management of LBP patients. Two-thirds of the chiropractors reported adhering to the guidelines regarding management and advice for LBP patients. Practitioners should improve guideline adherence, particularly for acute LBP cases, and when advising on return-to-work. (Source: Chiropractic and Manual Therapies)

Kinematics of the head and associated vertebral artery length changes during high-velocity, low-amplitude cervical spine manipulation
ConclusionsMean head angular displacements and VA length changes were small during CSM thrusts. Of the four different CSM measured, mean VA length changes were largest during rotation procedures. This suggests that if clinicians wish to limit VA length changes during the thrust phase of CSM, consideration should be given to the type of CSM used. (Source: Chiropractic and Manual Therapies)

Manual therapy versus advice to stay active for nonspecific back and/or neck pain: a cost-effectiveness analysis
ConclusionsOur results indicate that manual therapy for nonspecific back and/or neck pain is slightly less costly and more beneficial than advice to stay active for this sample of working age persons. Since manual therapy treatment is at least as cost-effective as evidence-based advice from a physician, it may be recommended for neck and low back pain. Further health economic studies that may confirm those findings are warranted.Trial registration Current Controlled Trials ISRCTN56954776. Systematic review of  guideline-recommended medications prescribed for treatment of low back pain
ConclusionsNine CPGs, included ten medication classes for the management of LBP. NSAIDs were the most frequently recommended medication for the treatment of both acute and chronic LBP as a first line pharmacological therapy. Acetaminophen and SMRs were inconsistently recommended for acute LBP. Meanwhile, with less consensus among CPGs, acetaminophen and antidepressants were proposed as second-choice therapies for chronic LBP. There was significant heterogeneity of recommendations within many medication classes, although oral corticosteroids, benzodiazepines, anticonvulsants, and antibiotics were not recommended by any CPGs...

Pressure pain thresholds in a real-world chiropractic setting: topography, changes after treatment, and clinical relevance?
ConclusionsThis study of real-world chiropractic patients failed to demonstrate a substantial local or generalized increase in PPT following a clinical encounter that included SMT. This runs counter to prior laboratory research and questions the generalizability of highly experimental setups investigating the effect of SMT on PPT to clinical practice. Exploring factors influencing chiropractors ’ adherence to radiographic guidelines for low back pain using the Theoretical Domains Framework
ConclusionWe identified potential influences, in six theoretical domains, on participating chiropractors ’ LBP imaging behaviours and adherence to radiographic guidelines. These beliefs may be targets for theory-informed behaviour change interventions aimed at improving these target behaviours for chiropractors in NL. (Source: Chiropractic and Manual Therapies)

The impact and public health response of chiropractors to the COVID-19 pandemic: a survey across four continents
ConclusionsChiropractors who responded to our survey made substantial infectious control changes in response to COVID-19. However, there was regional variation in the implementation of the advised practitioner and patient use of PPE and limited overall use of telehealth consultations by chiropractors during COVID-19. Musculoskeletal spine-care chiropractors were more adaptive to certain COVID-19 public health changes within their practice setting than subluxation-based chiropractors. Disability burden due to musculoskeletal conditions and low back pain in Australia: findings from GBD 2019
ConclusionsThe ongoing high burden due to musculoskeletal conditions impacts Australians across the life course, and in particular females and older Australians. Strategies for integrative and organisational interventions in the Australian healthcare system should support high-value care and address key modifiable risk factors for disability such as smoking, occupational ergonomic factors and obesity. I expected to be pain free: a qualitative study exploring athletes ’ expectations and experiences of care received by sports chiropractors
ConclusionsIn general, participants were very satisfied with care. Overall, participants ’ expectations and experiences aligned but changed over time. Addressing the findings of this study can be used to enhance the quality of care provided to athletes from sports chiropractors, as well as inform future research agendas. Further work assessing if athletes in other competitive levels h ave similar experiences and expectations is needed. (Source: Chiropractic and Manual Therapies)

Impact of contextual factors on patient outcomes following conservative low back pain treatment: systematic review
ConclusionThis review identified influential contextual factors which may augment conservative chronic low back pain care. The heterogeneity of interventions suggests modifying more than one contextual factor may be more impactful on patients ’ clinical outcomes, although these findings require judicious interpretation.
