Periprosthetic femoral fractures
Br J Hosp Med (Lond). 2024 Feb 2;85(2):1-9. doi: 10.12968/hmed.2023.0378. Epub 2024 Feb 27.ABSTRACTThe incidence of periprosthetic femoral fractures is rising in the UK, because of an ageing population and an increasing number of hip arthroplasty operations being performed. They can occur intra- or postoperatively, and usually follow low energy trauma. They present with pain, swelling over the thigh, and an inability to weight bear. Periprosthetic femoral fractures are usually classified as per the unified classification system. Their management usually is dependent on their classification, with type A (fracture at level o...

Martha's rule: applying a behaviour change framework to understand the potential of complementary roles of clinicians and patients in improving safety of patients deteriorating in hospital
CONCLUSIONS: The capability-opportunity-motivation behaviour change framework provides insights into the causes of failure to rescue in deteriorating patients and an argument for opportunities through escalation by patients and families through Martha's rule. This might reduce the number of system failures and enable safer care.PMID:38416522 | DOI:10.12968/hmed.2023.0422 (Source: British Journal of Hospital Medicine)

Health inequalities for people with learning disabilities: why it matters and what emergency physicians need to know
Br J Hosp Med (Lond). 2024 Feb 2;85(2):1-7. doi: 10.12968/hmed.2023.0357. Epub 2024 Feb 27.ABSTRACTPeople with learning disabilities die on average 16 years earlier than the general population in England. They are a vulnerable group and may have unhealthy lifestyles and multimorbidity that lead to poor health outcomes. Worryingly, premature deaths are also more common and these often have contributory healthcare causes. This may be a result of staff lacking awareness, expertise and experience managing people with learning disabilities, the lack of reasonable adjustments, or discriminatory attitudes. Other issues include po...

Paediatric supracondylar fractures: assessment and management
Br J Hosp Med (Lond). 2024 Feb 2;85(2):1-7. doi: 10.12968/hmed.2023.0112. Epub 2024 Feb 7.ABSTRACTSupracondylar fractures of the distal humerus are the most common fracture around the elbow in children. A thorough initial assessment must be conducted to identify any associated neurovascular injury and carefully documented. The assessment should include a vascular examination of the radial pulse, temperature, colour and capillary refill time. A neurological examination must comment on the motor and sensory function of the radial, median and ulnar nerves. X-rays allow an evaluation of the fracture location and type, and the ...
