Gut Microbiota Perturbations in Reactive Arthritis and Postinfectious Spondyloarthritis
ConclusionThis is the first culture‐independent study characterizing the gut microbial community in postinfectious arthritis. Although bacterial factors correlated with disease presence and clinical features of ReA, host genetics also appeared to be a major independent driver of intestinal community composition. Understanding of these gut microbiota–host genetic relationships may further clarify the pathogenesis of postinfectious spondyloarthritides. (Source: Arthritis and Rheumatism)

Cancer immunotherapy in patients with preexisting rheumatologic disease: the Mayo Clinic experience
ConclusionTo our knowledge, this represents the largest single‐center cohort of patients with rheumatologic diseases who were exposed to modern cancer immunotherapy. Only a minority of these patients experienced a flare of their preexisting rheumatologic disease or any other IRAE.This article is protected by copyright. All rights reserved. Blood T Follicular Regulatory Cells / T Follicular Helper Cells ratio Marks Ectopic Lymphoid Structure Formation and PD ‐1+ICOS+ T Follicular Helper Cells Indicate Disease Activity in Primary Sjögren's Syndrome
ConclusionsBlood Tfr/Tfh ratio and PD‐1+ICOS+ Tfh cells constitute potential novel biomarkers for different features of primary SS. While blood Tfr/Tfh ratio is associated with ectopic lymphoid neogenesis, activated Tfh cells indicate disease activity. This article is protected by copyright. All rights reserved. Efficacy and safety of cyclophosphamide treatment in severe juvenile dermatomyositis shown by marginal structural modelling
ConclusionCYC is efficacious with no short‐term side‐effects seen in this study. Improvements in skin, global and muscle disease were observed. Further studies are required to evaluate longer‐term side‐effects. This article is protected by copyright. All rights reserved. (Source: Arthritis and Rheumatism)

Switched Memory B Cells Are Increased In Oligoarticular And Polyarticular Juvenile Idiopathic Arthritis And Their Change Over Time Is Related To Response To TNF Inhibitors
ConclusionsSwM B‐cells expand during disease course in JIA patients at a faster rate than in healthy children. This increase is more evident in early‐onset JIA; TNFi treatment inhibits this increase in patients who achieve and maintain remission, but not in those with active diseaseThis article is protected by copyright. Effects of HLA ‐B27 on Gut Microbiota in Experimental Spondyloarthritis Implicate an Ecological Model of Dysbiosis
ConclusionEffects of HLA‐B27 on gut microbiota and dysbiosis in SpA are highly dependent on host genetic background and/or environment despite convergence of dysregulated immune pathways. These results indicate an ecological model of dysbiosis where the effects of multiple microbes contribute to the aberrant immune response, rather than a single or small number of microbes driving pathogenesis. This article is protected by copyright. Belimumab for the Treatment of Early Diffuse Systemic Sclerosis
ConclusionPatients in both treatment groups experienced significant improvements in MRSS. The median difference was greater in the belimumab group but did not achieve statistical significance in this small pilot study. AEs were similar between the groups. Changes in gene expression were consistent with mechanism of action and showed that clinical response to treatment with belimumab is associated with a significant decrease in profibrotic genes and pathways. Additional studies are needed to determine the role of belimumab in the treatment of dcSSc. (Source: Arthritis and Rheumatism)

Loading ‐Induced Reduction in Sclerostin as a Mechanism of Subchondral Bone Plate Sclerosis in Mouse Knee Joints During Late‐Stage Osteoarthritis
ConclusionSince focal stress on the SBP underlying sites of cartilage damage increases during late stages of OA, these findings establish mechanical loading–induced attenuation of sclerostin expression and elevation of bone formation along the SBP surface as the major mechanisms characterizing subchondral bone phenotypes associated with severe late‐stage OA in mice. High levels of DEK autoantibodies in sera of polyarticular JIA patients and in early flare following cessation of anti ‐TNF therapy
ConclusionsDEK autoantibody levels are higher in patients with polyarticular, rather than oligoarticular, JIA and in patients who flare after cessation of anti‐TNF therapy. The C‐terminal 25 aa is the most immunogenic portion of DEK. These findings are of significance with respect to the nature of DEK autoantibodies and their contribution to the pathogenesis and management of JIA.This article is protected by copyright. Hip and knee arthroplasty utilization and outcomes in the United States and Canada: an analysis of New York and Ontario administrative data
ConclusionOntario has higher TJA utilization than NY, but a smaller percentage of hospitals performing these procedures. Patients are more likely to be discharged home and less likely to be readmitted in Ontario. Our results suggest areas where each jurisdiction could improve. This article is protected by copyright.
