Highlights from the literature
More treatment for obesity These are adult data but worthwhile reading. Tirzepatide, which is used in type two diabetes and is an insulinotropic polypeptide and glucagon-like peptide-1 receptor agonist has not been examined in obese adults without diabetes. It is a once-a-week subcutaneous delivered medication. Jastreboff AM et al [N Engl J Med 2022; 387:205–216 DOI: 10.1056/NEJMoa2206038] have completed a phase three double-blind, randomised, controlled trial in 2539 adults with a body-mass index of 30 or more, or 27 or more and at least one weight-related complication, excluding diabetes over 72 weeks. They have lo...

Are Britains rivers safe for family recreational activities in 2023?
We have fond memories of spending our family summers playing in British rivers. However, the Environmental Agency (EA) highlights alarmingly frequent and prolonged discharges of untreated sewage into our rivers, through storm overflow spills1—averaging 825 spills each day, totalling nearly 1.8 million hours over 2022. Concerns must be raised whether all of these releases are necessary, and why none of our rivers now meet legal water quality standards. Clean water and sanitation is one of the most important modern public health interventions. As human excrement contains over 50 pathogens,2 the accidental ingestion of ...

Socioeconomic barriers preventing children and young people living with long-term health conditions from achieving optimal outcomes
Children and young people living with a chronic illness (CI) and faced with poverty are less likely to benefit from medical advances than their more affluent peers.1–3 Clinical outcomes are poorer,1–4 setting these children on a trajectory of lifelong disadvantage. Inequality may be due to a variety of factors: some require action at a political level, while others could be addressed closer to home. In the Liverpool City Region, more than 80 000 children and young people are growing up in poverty. We undertook a scoping exercise in collaboration with Health Junction, an independent, not-for-profit community int...

Access to evidence-based treatments for young people with body dysmorphic disorder
Introduction Body dysmorphic disorder (BDD) typically emerges during adolescence, affects approximately 2% of the general population and is highly impairing. Despite its prevalence and impact, awareness of BDD remains poor and the condition often goes undiagnosed and untreated.1 Cognitive–behavioural therapy (CBT) and selective serotonin reuptake inhibitors (SSRIs) are effective treatments for BDD and recommended as the first-line interventions.2 However, little is known about the accessibility of these treatments within the UK or among young people specifically. We conducted a case note review to examine the treatme...

The SCOUT study finally arrives
This study started recruiting in 2012 from two children’s hospital in Philadelphia and Pittsburgh and it took 7 years to recruit. Congratulations to the researchers. It illustrates, that to deliver a rigourous well managed, randomised controlled trial in children with a common clinical problem, it takes time and commitment. Zaoutis T et al (JAMA Pediatr doi:10.1001/jamapediatrics.2023.1979) have tackled the topic of antibiotic treatment for uncomplicated urinary tract infection (UTI) in children. How long should we treat for? They have compared the efficacy of standard-course (10 days) and short-course therapy (5 day...

Quality of life of children and young people with anterior chest wall deformity: a systematic review of the literature
Conclusions Correction of ACWD is associated with significant improvement in the psychosocial QoL of children and young people. Further work is required to standardise QoL data collection for all children with ACWD to achieve a greater understanding of the impact and guide future management. Effectiveness of asthma preventer dispensing for preventing childhood asthma readmissions: a multisite cohort linkage study
Conclusions There was a low rate for prescribing and dispensing of hospital discharge asthma preventers and no protective effect was found for its impact on readmissions. A protective effect on readmissions was found for community asthma preventer dispensing. (Source: Archives of Disease in Childhood)

Assessing maternal alcohol consumption in pregnancy: comparison of confidential postnatal maternal interview and measurement of alcohol biomarkers in meconium
Conclusions FAEEs and EtG measured in meconium have low sensitivity and specificity for self-reported alcohol consumption after 20 weeks’ gestation in an unselected Scottish population. (Source: Archives of Disease in Childhood)

Navigating the unknown: understanding and managing parental anxiety when a child is ill
Conclusions Although parents make carefully considered decisions on whether or not to consult a doctor, many appear to miss red flags, including more experienced parents. Conversely, some become overly concerned with certain specific symptoms such as fever, and few parents are familiar with self-management strategies. (Source: Archives of Disease in Childhood)

Association between maternal influenza vaccination and neurodevelopmental disorders in childhood: a longitudinal, population-based linked cohort study
Conclusions We did not observe increased risk of neurodevelopmental disorders following in utero exposure to seasonal IIV. Although we observed some evidence for lower risk of seizure and sleep disorders, additional studies are required to confirm. (Source: Archives of Disease in Childhood)

Pachydermodactyly
A 15-year-old boy presented with swelling in his fingers, which had developed over 3 years. He had no history of trauma, pain and fever or morning stiffness. He was right-handed, with anxiety issues and also had repetitive finger friction behaviour. On examination, soft tissue swelling and skin thickening were observed in the lateral of the bilateral second to fourth proximal interphalangeal (PIP) joints bilaterally and left-hand second distal interphalangeal joints (figure 1). Complete blood counts, biochemistry and inflammatory marker levels were normal. Antinuclear antibody, rheumatoid factor and cyclic citrullinated pe...

Are children with prolonged fever at a higher risk for serious illness? A prospective observational study
Conclusion Children with prolonged fever have a higher risk of SBI, warranting a careful clinical assessment and diagnostic workup. Warning signs of SBI occurred infrequently but, if present, increased the likelihood of SBI. Although rare, clinicians should consider important non-infectious causes of prolonged fever. Neurodevelopment, vision and auditory outcomes at age 2 years in offspring of participants in the 'Women First maternal preconception nutrition randomised controlled trial
Conclusions Prenatal maternal nutrition supplementation was not associated with any neurodevelopmental outcomes at age 2 years. Maternal education, family environment and LAZ24 predicted ECD. Interventions addressing multiple components of the nurturing care model may offer greatest impact on children’s developmental potential. Trial registration number NCT01883193.
