Effect of systemic hydrocortisone in ventilated preterm infants on parent-reported behavioural outcomes at 2 years corrected age: follow-up of a randomised clinical trial
Conclusion This study found high rates of behaviour problems at 2 years’ CA following very preterm birth, but these problems were not associated with hydrocortisone treatment initiated between 7 and 14 days after birth in ventilated preterm infants. Trial registration number NTR2768; EudraCT 2010-023777-19. (Source: Archives of Disease in Childhood - Fetal and Neonatal Edition)

Survey of transfusion practices in preterm infants in Europe
Conclusions Transfusion thresholds and aspects of administration vary widely across European NICUs. In general, transfusion thresholds used tend to be more liberal compared with data from recent trials supporting the use of more restrictive thresholds. Further research is needed to identify the barriers and enablers to incorporation of recent trial findings into neonatal transfusion practice. Birth order and morbidity and mortality to hospital discharge among inborn very low-birthweight, very preterm twin infants admitted to neonatal intensive care: a retrospective cohort study
Conclusion In modern perinatology, second twins are still more unstable immediately after birth and require more resuscitation. After admission to the neonatal intensive care unit, they are at increased risk of RDS, but not other conditions, except for second twins delivered by C-section after a first twin delivered vaginally, who have decreased overall survival and survival without major brain injury. Emotional journey of Asian mothers of premature infants who received pasteurised donor human milk: a qualitative study
Conclusion Mothers undergo a spectrum of complex emotions from initial hesitation to acceptance with gratitude, when their premature infants receive donor human milk. Some continue to struggle with negative emotions and require more support. By recognising their emotional responses, healthcare providers can support mothers in their breastfeeding journey with targeted counselling. Thresholds for surfactant use in preterm neonates: a network meta-analysis
Conclusion Our results do not show a clear benefit of surfactant treatment at any threshold of FiO2. The 60% threshold was suggestive of increased morbidity. There was no advantage seen with prophylactic treatment. Randomised trials of different thresholds for surfactant delivery are urgently needed to guide clinicians and provide robust evidence. Congenital myofibroma of the hand
A term newborn presented with a red-violaceous mass on the left hand encompassing the thenar eminence (figure 1). Ultrasonography showed a solid, slightly vascularised lesion involving the first metacarpal. MRI showed a solid, hypovascular mass with an area of necrosis infiltrating and involving the first metacarpal (figure 2A). A biopsy of the lesion was performed, which confirmed that it was a benign mesenchymal tumour compatible with myofibroma. A watchful waiting attitude with annual MRI controls was indicated, and the lesion involuted remarkably during the following years until it practically disappeared, with residua...

Getting to health equity in NICU care in the USA and beyond
Differences in race/ethnicity, gender, income and other social factors have long been associated with disparities in health, illness and premature death. Although the terms ‘health differences’ and ‘health disparities’ are often used interchangeably, health disparities has recently been reserved to describe worse health in socially disadvantaged populations, particularly members of disadvantaged racial/ethnic groups and the poor within a racial/ethnic group. Infants receiving disparate care based on race/ethnicity, immigration status, language proficiency, or social class may be discomforting to hea...

When to treat with surfactant?
Synthesis of available data by different expert groups has resulted in differing fraction of inspired oxygen (FiO2) thresholds for surfactant therapy in babies with respiratory distress syndrome (RDS). Canadian Guidelines in 2021 suggested treatment when FiO2>0.5. Recent European Guidelines suggest treatment when FiO2>0.3. Branagan et al attempt through network meta-analysis to determine if there is an optimal FiO2 cut-off for surfactant therapy to reduce any of the important negative outcomes of prematurity including mortality and bronchopulmonary dysplasia (BPD). They show that there are no strong data from randomi...

Fantoms
When to treat with surfactant? More than 30 years after surfactant treatment came into widespread use for preterm infants there is still a lot of variation in practice. Thresholds for treatment in infants who are stabilised on non-invasive respiratory support vary widely. Having been involved in discussions about trial protocols relating to respiratory support and outcomes I know that our long and tortuous journey of treatment preferences means that this is an area where there are strongly held views and limited equipoise. Aoife Branagan and colleagues report a network meta-analysis of randomised trials of different treatm...

Reliability of respiratory function monitor interpretation for neonatal resuscitation
A respiratory function monitor (RFM) is a potential tool to enhance the quality of positive pressure ventilation (PPV) inflations in the delivery room.1 The RFM calculates, displays and records data about tidal volumes, mask leak, airway obstruction and presence of spontaneous breathing. Although visual interpretation of respiratory signals is integral to this methodology, the reliability of these assessments is unknown. The study objectives were to determine the intrarater and inter-rater reliability of RFM signal interpretation. This was an observational study of RFM files recorded during PPV resuscitation of preterm inf...

Lactate acidosis and hypoglycaemia in twin anaemia polycythemia sequence donors
This study was set up to investigate the prevalence of lactate acidosis and hypoglycaemia in TAPS donors and recipients, to explore potential hypotheses for the difference in NDI. Monochorionic twins diagnosed with TAPS postnatally between 2002 and 2021 at our hospital were included. TAPS was defined as a postnatal haemoglobin... Resuscitation of a preterm infant with massive air embolism
Immediately after surfactant administration, a ventilated preterm male infant of 27 weeks g.a. developed massive pulmonary obstruction, apparent from the ventilator’s display, loss of pulse wave from the arterial umbilical catheter, severe bradycardia, extreme pallor of the legs and cyanotic appearance of the abdomen. Chest compression was started instantaneously, and emergency X-ray revealed massive air embolism with ingress of gas into all cardiac cavities and surrounding great vessels (figure 1). Immediate aspiration yielded 3 mL gas through the umbilical venous catheter, further 2 mL were drained by cardiocentesi...

Dexmedetomidine affects cerebral activity in preterm infants
The use of dexmedetomidine (DEX) has been extended in preterm newborns, but the effects on cerebral activity and their relationship with haemodynamic changes has not been studied. We retrospectively studied the effects of DEX administered to 10 preterm newborns, assessing amplitude-integrated EEG (aEEG) parameters, brain regional SO2 (brSO2), heart rate, non-invasive mean blood pressure (MBP), transcutaneous oxygen saturation (SpO2), venous pCO2 and haemoglobin (Hb) values, in two 6-hour periods: one starting 6 hours before the beginning of DEX perfusion and the other 6 hours afterwards. DEX infusion led to brSO2 decrease ...
