Neonatal BCG: a time for change
The BCG vaccination programme in the UK is risk based and has usually been given to eligible babies soon after birth. On advice from the Joint Committee on Vaccination and Immunisation, NHS England and Improvement recently revised the timing of this vaccination to 28 days after birth or soon thereafter. In this article, we highlight the change in timing of vaccination, the rationale and barriers to BCG uptake that this change may pose. Fifteen-minute consultation: Child with persistent runny nose
A child with persistent runny nose may cause significant parental anxiety and healthcare utilisation. While the most common diagnoses are recurrent acute viral upper respiratory tract infections and allergic rhinitis, a careful history and examination is necessary to exclude other causes and to identify comorbidities. Treatment can then be tailored to the underlying cause. The aim of this article is to provide a systematic approach to such patients. Epistle
As the year comes to a draw and we welcome the new year, we bring you another exciting issue. Hannah Walsh et al highlight how linguistic challenges can be overcome, to potentiate participation in research studies (see page 55). The authors elaborate on the issues involved, suggest potential solutions, and signpost useful existing guidance. Inclusion of a linguistic diverse population in research is absolutely critical for developing policies, tools and strategies to reduce health inequalities. By addressing language barriers, researchers empower diverse communities to be represented and therefore generate more robust evid...

Widespread rash, leg pain and increased muscle bulk
A 4-year-old girl presented with a 2-year history of rashes on both hands, trunk and knees and an inability to move the right knee with difficulty in walking. On examination, multiple hypopigmented confluent macules were present on the trunk (figure 1A), extensor surfaces of knees (figure 1B), on elbows (figure 1C) and dorsal surface of bilateral metacarpophalangeal and interphalangeal joints (figure 1D). The bulk of the right thigh and calf was increased compared with the left side and reflexes were 2+ throughout. Blood tests revealed an elevated level of creatine kinase (CK) (320 U/L), erythrocyte sedimentation rate was ...

How to solve a clinical conundrum: Have you tried a trio exome analysis?
The following report describes the clinical journey of a 5-month-old male infant who presented with a significant kidney injury following a diarrhoeal illness. His course was complicated by severe hypertension and a number of acute life-threatening events necessitating periods of time on the intensive care unit, where he received ventilatory support and underwent renal replacement therapy and treatment with a monoclonal antibody therapy. We take the reader on a stepwise journey from presentation through to final diagnosis, discussing important biochemical, haematological and radiological features where learning points are ...

Chloral hydrate - use as a sedative in paediatric settings
The aim of this article is to discuss chloral hydrate and its place in current therapy in various paediatric settings following the 2021 safety update from the Medicines and Healthcare products Regulatory Agency (MHRA). There are two case studies presented to apply learning relating to the place of chloral hydrate in both paediatric sedation and neurology. 2021 MHRA update When the MHRA issued a drug safety update in October 2021 relating to the use of chloral hydrate and its prodrug cloral betaine in paediatric patients, it sparked concern for families and the paediatric medical community.1 While no new safety concerns we...

Recommendations for minimising and monitoring adverse effects of long-term supraphysiological corticosteroid use in children and young persons
KEY POINTS Supraphysiological systemic corticosteroids are used commonly in the management of various medical conditions including in children and young persons. These have several side-effects with growth suppression and pubertal delay being specific to this population. Prior to initiating long-term corticosteroids, it is important to take a detailed history and do a physical examination including growth and nutritional statuses, blood pressure and pubertal staging, and do baseline blood investigations as listed in the main text. Although the risks cannot be eliminated completely, there are multiple strategies a clinician...

Medical emergencies in eating disorders
About the guideline Eating disorders are serious mental health disorders that cannot be appropriately managed without both physical and mental healthcare. These illnesses, including anorexia nervosa, have the highest avoidable mortality rates of all mental health conditions. Paediatricians should be aware that the risk of death in children with eating disorders is comparable to that of asthma and diabetes.1 The COVID-19 pandemic and its aftermath have had a significant impact in terms of increased incidence as well as acuity of presentations of children with eating disorders, which has contributed to a 90% increase in hosp...
