Management of cancer-associated thromboembolism in vulnerable population
Arch Cardiovasc Dis. 2023 Nov 23:S1875-2136(23)00518-1. doi: 10.1016/j.acvd.2023.11.009. Online ahead of print.ABSTRACTAlthough all patients with cancer-associated thrombosis (CAT) have a high morbidity and mortality risk, certain groups of patients are particularly vulnerable. This may expose the patient to an increased risk of thrombotic recurrence or bleeding (or both), as the benefit-risk ratio of anticoagulant treatment may be modified. Treatment thus needs to be chosen with care. Such vulnerable groups include older patients, patients with renal impairment or thrombocytopenia, and underweight and obese patients. Howe...

Central venous catheter associated upper extremity deep vein thrombosis in cancer patients: Diagnosis and therapeutic management
This article addresses the issues of diagnosis and management of CRT through a review of the available literature and makes a number of proposals based on the available evidence. In symptomatic patients, venous ultrasound is the most appropriate choice for first-line diagnostic imaging of CRT because it is noninvasive, and its diagnostic performance is high (which is not the case in asymptomatic patients). In the absence of direct comparative clinical trials, we suggest treating patients with CRT with a therapeutic dose of either a LMWH or a direct oral factor Xa inhibitor, with or without a loading dose. These anticoagula...

Cancer-related arterial thromboembolic events
Arch Cardiovasc Dis. 2023 Nov 23:S1875-2136(23)00516-8. doi: 10.1016/j.acvd.2023.11.007. Online ahead of print.ABSTRACTCancer is associated with a hypercoagulable state and is a well-known independent risk factor for venous thromboembolism, whereas the association between cancer and arterial thromboembolism is less well established. Arterial thromboembolism, primarily defined as myocardial infarction or stroke is significantly more frequent in patients with cancer, independently of vascular risk factors and associated with a three-fold increase in the risk of mortality. Patients with brain cancer, lung cancer, colorectal c...

Acute mitral isthmus block during catheter ablation with vein of Marshall ethanol infusion: Angiographic considerations
CONCLUSION: Ethanol infusion in the vein of Marshall, when feasible (82%), was a safe approach to obtaining a high success rate (94%) of acute bidirectional endocardial and epicardial mitral isthmus block.PMID:38040560 | DOI:10.1016/j.acvd.2023.11.001 (Source: Archives of Cardiovascular Diseases)

Bioprosthetic leaflet thrombosis and reduced leaflet motion after transcatheter aortic valve replacement: A systematic review and meta-analysis
CONCLUSIONS: The proportion of leaflet thrombosis is highly variable according to the screening approach, the type of valve and the use of oral anticoagulation. The occurrence of cerebral events is increased when leaflet thrombosis and/or reduced leaflet motion are diagnosed, but leaflet thrombosis has no impact on survival.PMID:37968182 | DOI:10.1016/j.acvd.2023.10.003 (Source: Archives of Cardiovascular Diseases)
