A large basal left ventricular pseudoaneurysm following pediatric cardiac surgery
Giuseppe Scrascia, Raffaella Lamparelli, Alberto Gaeta, Dario Troise, Giandomenico Stellacci, Ugo Vairo, Gabriele ScalzoAnnals of Pediatric Cardiology 2022 15(3):294-296 Left ventricular pseudoaneurysm (LV-PSA) is a rare complication in children, usually developing after cardiac surgery, percutaneous procedures, infections, or trauma. Herein, we report a case of large basal submitral LV-PSA in a 36-day-old baby, detected 26 days after cardiac operation for hypoplastic arch, aortic coarctation, and small ventricular septal defect. No complications occurred in the first postoperative course, and early postoperative echocard...

Left cervical and circumflex aortic arch with aberrant right subclavian artery arising from a Kommerell diverticulum: A rare vascular ring
We report a rare case of vascular ring due to a left cervical circumflex aortic arch with aberrant right subclavian artery arising from a Kommerell diverticulum. This is a very rare case of vascular ring in a newborn with stridor and stenosis of the left pulmonary artery. A cardiac catheterization and an angio-computed tomography scan were helpful to clarify the diagnosis. The surgical correction was performed with division of the arterial duct, reimplantation of the right subclavian artery, aortic and tracheal suspension, and pulmonary arterial plasty. The patient was asymptomatic at discharge and 1-year follow-up. (Sourc...

Myocarditis in children after COVID-19 vaccine
Othman A Aljohani, Lisa Arcilla, Neeru Kaushik, Nicole M Cresalia, Boran Li, April A Edwell, Alice M Ramirez, Shafkat AnwarAnnals of Pediatric Cardiology 2022 15(3):280-283 Three healthy adolescents presented with myocarditis confirmed on cardiac magnetic resonance imaging after receiving Pfizer-BioNTech COVID-19 vaccine. All patients were hemodynamically stable and had good short-term outcomes. Long-term outcomes are yet to be determined. Larger studies are needed to determine whether an association between Pfizer-BioNTech COVID-19 vaccine and myocarditis exists. (Source: Annals of Pediatric Cardiology)

Strategies to mitigate inflammation in management of complex congenital heart disease complicated by & #8220;multisystem inflammatory syndrome in children & #8221;
Anil Kumar, Reena K Joshi, Neeraj Aggarwal, Mily Ray, Raja JoshiAnnals of Pediatric Cardiology 2022 15(3):276-279 A 6-month-old boy, a case of Shone's complex, presented in decompensated state was found to have severe mitral stenosis along with multisystem inflammatory syndrome in children (MISC) warranting urgent surgical intervention. Various modalities including cytokine-adsorbing hemofilter were used to target inflammation. Postoperatively, the child recovered from low cardiac output accompanied by decrease in the levels of inflammatory markers, inopressors, and ventilatory requirements. Open heart surgery ...

Oncogenesis in patients with congenital heart disease: A possible role of the neural crest
We report a series of patients, followed in two high-volume referral centers, with CHD involving neural crest-derived structures who developed cancer later in life. Fourteen patients (five female) developed neoplasia with a cellular origin embryologically linked to the neural crest between 2010 and 2020. If confirmed on larger datasets, this observation might support the hypothesis of common embryogenetic pathway suggesting tailored surveillance of a specific subset of patients. Is there an association of near-infrared spectroscopy with low cardiac output and adverse outcomes in single-ventricle patients after stage 1 palliation ?
Conclusions : Cerebral NIRS ≤57% at 6 h after surgery detected LCO after stage 1 palliation in single-ventricle patients. Cerebral or renal NIRS was not associated with adverse outcomes and therefore, may not be useful in predicting adverse outcomes in this population. (Source: Annals of Pediatric Cardiology)

Outcomes of primary repair of sternal cleft defects: Providing a & #8220;bony cover & #8221;
Conclusion : Bony cover to the heart can be provided in all varieties of sternal cleft defects using primary surgical repair early in infancy. The delay in surgical correction increases the complexity of the procedure and may require the use of prosthetic material which has its own disadvantages. (Source: Annals of Pediatric Cardiology)

Fontan procedure on deep hypothermic circulatory arrest: Short-term results and technique
Conclusions : Fontan procedure can be safely performed on CPB and DHCA with good operative results. This operative strategy may be used in special circumstances like in patients with situs and systemic venous anomalies and those requiring repair of a complex intracardiac defect. Long-term follow-up will be required to evaluate if this strategy has any impact on the neurodevelopmental outcome and the long-term sequelae of Fontan. (Source: Annals of Pediatric Cardiology)

Predictors of mortality and morbidity in total anomalous pulmonary venous connection with biventricular physiology: A 10-year Indian single centre experience of 492 patients
Conclusions : Although peri-operative TAPVC mortality has reduced, mortality on follow-up continues to occur and is partly due to the obstruction of pulmonary venous pathway. Meticulous follow-up holds the key in further reducing the mortality. Larger studies are needed for the identification of risk factors for pulmonary venous obstruction and its preventive strategies. Aortopulmonary artery fistula: A rare complication of balloon dilatation of the pulmonary artery
We present a case of an 8-year-old female with a history of complex Taussig-Bing anomaly, who underwent an arterial switch procedure with LeCompte maneuver and ventricular septal defect closure early in life. The patient developed neopulmonary stenosis and branch pulmonary artery (PA) stenosis, for which she underwent patch augmentation and balloon dilatation of the left PA. The patient presented with a fistula between the ascending aorta and the left branch PA, confirmed by echocardiography and cardiac catheterization. She underwent repair of the APF with a homograft patch reconstruction of the ascending aorta. (Source: A...

Transcatheter management of proximal stent migration in coarctation of the aorta
Diogo Remi Oliveira Faim, Patrícia Vaz Silva, Andreia Francisco, António PiresAnnals of Pediatric Cardiology 2022 15(2):222-224 Proximal stent migration in setting of transcatheter management of coarctation of the aorta is a rare complication, which may require emergency surgery. Herein, we report a successful transcatheter management of proximal stent migration in a high surgical risk, a 14-year-old girl, which caused partial occlusion of both the descending aorta and the left subclavian artery. Spontaneous closure of patent ductus arteriosus in preterm babies after failed attempts at transcatheter device closure
We report three babies in whom transcatheter closure of PDA was unsuccessful; however, the PDA closed spontaneously in 1–5 days after the transcatheter attempt. The PDA remained closed during follow-up at 3 weeks, 10 weeks, and 17 months, respectively. Mechanical stimulation of the PDA by the wire during attempted device closure likely induced the PDA closure. Further studies are needed to evaluate whether this is a useful alternative therapy in this patient population. (Source: Annals of Pediatric Cardiology)
