Hospitalizations for Cardiovascular Diseases Sensitive to Primary Health Care in Paran á State, Brazil: A Bayesian Spatiotemporal Model
CONCLUSIONS: The annual risk of hospitalization decreased over time; however, small municipalities had the greatest rates of hospitalization, indicating an increase in health inequity. The inverse association between social development and hospitalizations for CVD sensitive to PHC raises questions about intersectionality in health care.PMID:38527827 | DOI:10.1370/afm.3083 (Source: Annals of Family Medicine)

Mission Impossible? Managing the American Academy of Pediatrics' Obesity Guideline
Ann Fam Med. 2024 Mar-Apr;22(2):170-171. doi: 10.1370/afm.3069.ABSTRACTA primary care pediatrician casts a skeptical eye at the American Academy of Pediatrics Obesity Guideline. Using back-of-the-envelope calculations, she explains that meeting the guidelines would swamp her office, hospital, and the country's clinicians in a manner that is unrealistic. Warning against the alienation that boots-on-the-ground clinicians experience when guidelines are too theoretical to be practical, she suggests alternative avenues for addressing this public health issue.PMID:38527829 | DOI:10.1370/afm.3069 (Source: Annals of Family Medicine)

Joe's Story: How a Capitated Payment Model Lets Me Be the Physician I Want to Be
Ann Fam Med. 2024 Mar-Apr;22(2):167-169. doi: 10.1370/afm.3070.ABSTRACTFor many years I cared for Joe, following him through diagnoses of strokes, end-stage renal disease, and metastatic prostate cancer. Gaining his trust, coordinating his care across specialist visits and hospitalizations, and helping him and his family clarify goals of care took an investment of time and relationship-building. I was able to spend this time with Joe, and all of my medically complex patients, because I had taken a job in a Program of All-Inclusive Care for the Elderly (PACE), a fully capitated model of care. With care organized around the ...

"It Can't Hurt!": Why Many Patients With Limited Life Expectancy Decide to Accept Lung Cancer Screening
CONCLUSIONS: Patients in this study with multimorbidity and limited life expectancy were unaware of their greater risk of potential harm when accepting LCS. Given patient trust in clinician recommendations, it is important that clinicians engage patients with marginal LCS benefit in shared decision making, ensuring that their values of desiring more information about their health are weighed against potential harms from further evaluations.PMID:38527813 | DOI:10.1370/afm.3081 (Source: Annals of Family Medicine)

Small Independent Primary Care Practices Serving Socially Vulnerable Urban Populations
CONCLUSIONS: Small independent practices serving vulnerable patients in urban communities are surviving because deeply committed physicians are making personal sacrifices. Health equity-focused policies could decrease the burden on these physicians and bolster independent practices so that socially vulnerable patients continue to have options when seeking primary care.PMID:38527816 | DOI:10.1370/afm.3068 (Source: Annals of Family Medicine)

Genital Tucking Practices in Transgender and Gender Diverse Patients
Ann Fam Med. 2024 Mar-Apr;22(2):149-153. doi: 10.1370/afm.3076.ABSTRACTGenital tucking (tucking) is the practice of hiding or minimizing the appearance of one's genitals and gonads. We aimed to better understand the prevalence of tucking and its potential effect on behavior and health. An online questionnaire was distributed to adults with a diagnosis of gender dysphoria or gender incongruence (n = 98). The risk of side effects increased with the length of tucking sessions (P = 0.046) with many patients avoiding medical care despite experiencing side effects. Health care providers should empathetically discuss tucking and ...

Challenges Addressing Lung Cancer Screening for Patients With Multimorbidity in Primary Care: A Qualitative Study
CONCLUSIONS: Shared decision-making conversations about lung cancer screening differed substantially from the standard for patients with complex comorbidities. What AHRQ Learned While Working to Transform Primary Care
Ann Fam Med. 2024 Mar-Apr;22(2):161-166. doi: 10.1370/afm.3090.ABSTRACTBuilding on previous efforts to transform primary care, the Agency for Healthcare Research and Quality (AHRQ) launched EvidenceNOW: Advancing Heart Health in 2015. This 3-year initiative provided external quality improvement support to small and medium-size primary care practices to implement evidence-based cardiovascular care. Despite challenges, results from an independent national evaluation demonstrated that the EvidenceNOW model successfully boosted the capacity of primary care practices to improve quality of care, while helping to advance heart he...
