Effect of intravenous levosimendan or milrinone on left atrial pressure in patients undergoing off-pump coronary artery bypass grafting-A prospective double-blind, randomized controlled trial
Conclusion: Levosimendan may be used as a primary inotrope in terms of better reduction in left atrial pressure during different stages of OPCAB, translating to a decrease in left ventricular end-diastolic pressure, therefore maintaining optimum coronary perfusion pressure, which is the primary goal of the surgery.PMID:37861575 | PMC:PMC10691577 | DOI:10.4103/aca.aca_51_23 (Source: Annals of Cardiac Anaesthesia)

Comparison of success rate of abducted and neutral arm position for right infraclavicular subclavian vein cannulation under real-time ultrasound guidance in patients undergoing elective neurosurgery under general anesthesia
CONCLUSION: For USG-guided infraclavicular subclavian vein cannulation, abducted arm position, and neutral arm position in terms of first attempt success rate, number of attempts and associated complications has comparable results; however, further studies with larger group of patients are required to assess the overall advantage of abducted arm position over neutral arm position.PMID:37861576 | PMC:PMC10691558 | DOI:10.4103/aca.aca_32_23 (Source: Annals of Cardiac Anaesthesia)

Ultrasound: A novel alternative technique for cervical epidural space visualization-A pilot study
CONCLUSION: The pilot study has successfully demonstrated the implication of US for visualizing and aiding in epidural space catheterization. Also, the failure rate and procedural complications were drastically minimized with the help of US as compared to the traditional blind technique.PMID:37861577 | PMC:PMC10691571 | DOI:10.4103/aca.aca_40_23 (Source: Annals of Cardiac Anaesthesia)

Absent right superior vena cava and persistent left superior vena cava: An incidental finding
Ann Card Anaesth. 2023 Oct-Dec;26(4):433-434. doi: 10.4103/aca.aca_91_23.ABSTRACTA patient with Marfan syndrome undergoing Bentall operation was found to have an absent right superior vena cava and persistent left superior vena cava. The dilation of coronary sinus raised the suspicion of persistent left superior vena cava. The diagnosis was confirmed by agitated saline contrast echocardiography and computed tomography of the chest.PMID:37861579 | PMC:PMC10691572 | DOI:10.4103/aca.aca_91_23 (Source: Annals of Cardiac Anaesthesia)

Abnormally low regional cerebral oxygen saturation after induction of anesthesia without neurological abnormality: A case report
Ann Card Anaesth. 2023 Oct-Dec;26(4):442-445. doi: 10.4103/aca.aca_194_22.ABSTRACTMeasurement of regional cerebral oxygen saturation (rSO2) using near-infrared spectroscopy (NIRS) in cardiac surgery is known to be useful in reducing postoperative neurological complications. We here present a case of a 71-year-old man in whom severe decrease in unilateral rSO2 was observed after induction of general anesthesia for percutaneous mitral valve clipping, although no neurological abnormalities were found. NIRS does not always predict postoperative neurological complications.PMID:37861582 | PMC:PMC10691559 | DOI:10.4103/aca.aca_19...

Apnea management during WATCHMAN device deployment with apneic oxygenation: A case report of three cases
We present three cases in which apneic oxygenation was employed to maintain oxygenation during apnea, and oxygen reserve index (ORiTM) was measured to evaluate its safety and efficacy. Oxygen was administered continuously via the endotracheal tube during apnea. During all four apneic events in three patients (mean duration of 356 seconds), the ORi values maintained above 0.24, which is generally considered the threshold of partial pressure of arterial oxygen (PaO2) > 100 mmHg. Transcutaneous oxygen saturation and PaO2 remained above 99% and 300 mmHg, respectively. There were no respiratory or circulatory complications d...

Complex cyanotic congenital heart disease and cervical spine surgery: Ticking all the right boxes
Ann Card Anaesth. 2023 Oct-Dec;26(4):461-463. doi: 10.4103/aca.aca_25_23.ABSTRACTAlthough complex congenital heart disease (CHD) patients usually present in childhood, it is not rare to see adults well past middle age. These patients undergo continuous pathophysiological changes in their heart and blood vessels, making anesthetic management more challenging if surgery is required. Herein, we report a case in which understanding the anatomy and pathophysiology helped optimally manage a patient with a double outlet right ventricle (DORV) who underwent plating and fixation for a hangman's fracture in the prone position.PMID:3...

The protekduo cannula for acute right ventricular support in thyrotoxicosis
Ann Card Anaesth. 2023 Oct-Dec;26(4):464-467. doi: 10.4103/aca.aca_26_23.ABSTRACTA 25-year-old female was presented with acute right heart failure (aRHF) and cardiogenic shock secondary to thyrotoxicosis with concomitant acute respiratory failure. A ProtekDuo cannula was placed to provide temporary percutaneous right ventricular assistance and extracorporeal membrane oxygenation (ECMO) in venopulmonary (V-P) configuration, which provided both decompression of the right ventricle (RV) and oxygenation. With treatment of the underlying thyrotoxicosis, the RV function improved and respiratory failure resolved. She was discharg...
