Balancing benefits and potential harms of stock inhaler policies in schools
In this issue of Annals of Allergy, Asthma& Immunology, Hardy et al1 provide valuable insights into the feasibility and acceptability of common canister policies in schools. Schools with stock inhaler programs may be able to optimize point-of-care access to asthma rescue medications for both children and adults working in educational environments when needed while minimizing costs.2 The authors surveyed both school nurses and administrators, finding that less than 25% of those surveyed reported having stock inhaler programs with even lower adoption in more rural regions. Specific IgE point-of-care testing is evolving
There was a time when an Allergist, often with the collaboration of a local botanical specialist, would venture into his/her local woods and fields collecting pollen from local trees and weeds. Gradually, using percutaneous testing with solutions made from this collected pollen, allergists became aware of plants most likely to cause allergic symptoms. This testing was very specific for local flora. The article by Davies et al1 in this issue of Annals of Allergy, Asthma& Immunology begins to move back toward local testing, but using novel methods. The paradigm shift in anaphylaxis management cannot occur without changing minds
The paradigm shift in anaphylaxis management was fomented by the recent SARS-CoV-2 global pandemic. These recent uncertain times forced all of us to reconsider if, when, and why we should visit the emergency department or hospital. We suddenly had to weigh the risks of exposure to a potentially deadly virus (particularly before the availability of vaccines) with the necessity for urgent evaluation and treatment of various symptoms and conditions. Within our specialty, this prompted several experts to critically evaluate the evidence surrounding anaphylaxis management and whether our patients truly needed to go to the emerg...

'Tis the season for dupilumab?
Peters et al1 explored the efficacy of dupilumab in reducing asthma exacerbations across seasons in patients with asthma and type 2 inflammation, with and without an allergic phenotype, using post hoc data analysis from the LIBERTY ASTHMA QUEST (NCT02414854) and LIBERTY ASTHMA TRAVERSE (NCT02134028) studies. The investigators concluded that dupilumab similarly reduced severe exacerbations compared with placebo in treated patients independent of season, allergic sensitization, and baseline inhaled corticosteroid dose, supporting the use of dupilumab across seasons for all patients with type 2 asthma. How does pollution worsen allergies?
There is increasing evidence that pollution contributes to the rise in allergic diseases.1-3 Major pollutant contributors to this trend include particulate matter (PM), greenhouse and other harmful gases, and pollen.1-9 The studies conducted by various research groups have advanced our understanding of mechanisms by which pollution worsens allergies. The PM, in particular, is a complex mixture of components, and exposure to PM has been reported to worsen preexisting allergic disease symptoms.1,3,4 Furthermore, PM2.5, particles with a diameter of less than 2.5 mm, is considered the most harmful because of its ability to tra...
