Estimated burden of Chlamydia trachomatis female infection and consequent severe pelvic inflammatory disease, Italy, 2005-2016.
Authors: Sabbatucci M, Salfa MC, Regine V, Pezzotti P, Suligoi B Abstract Chlamydia trachomatis (Ct) is the leading sexually transmitted infection (STI) across Europe. In Italy, Ct prevalence is low in general population, but predominance of asymptomatic infections, passive voluntary reporting, variable diagnostic criteria and coding practices can lead to considerable underestimation, preventing assessment of real burden of disease and health intervention. We analysed data on female genital Ct infection registered in STI sentinel surveillance systems in Italy from 2005 through 2016 and found 3305 women. Among them,...

Value based healthcare for rare diseases: efficiency, efficacy, equity.
Authors: Fantini B, Vaccaro CM Abstract The paper reviews the literature available on value based health care and relates it to rare diseases. Starting from the economic definition of value and healthcare evaluation, efficacy and efficiency, it includes the equity dimension to define value-based healthcare. It embraces also the cultural framework associated to the concepts of health and disease, normal and pathological, right or wrong for the patient. The paper highlights that a prevention and recovery view and global evaluation of costs/benefits ratio for rare diseases make difficult and limited the applicability ...

Primary prevention as an essential factor ensuring sustainability of health systems: the example of congenital anomalies.
Authors: Taruscio D, Bermejo-Sánchez E, Salerno P, Mantovani A Abstract Protection of early development contributes to health of next generations. Congenital anomalies (and other adverse reproductive outcomes) are an important public health issue and early indicator of public health risks, as early development is influenced by many risk factors (e.g., nutrition, lifestyles, pollution, infections, medications, etc). Effective primary prevention requires an integrated "One Health" approach, linking knowledge and action. This requires surveillance of health events and potential health-damaging factors...

Health systems for rare diseases: financial sustainability.
Authors: Iskrov G, Stefanov R, Ferrelli RM Abstract INTRODUCTION: Distribution of public spending on health depends on a variety of factors, from disease burden and system priorities to organisational aspects and costs. Nowadays, virtually all health care systems face serious sustainability challenges. This is particularly true for rare diseases, where priority setting involves complex and often controversial value-laden choices. METHOD: The theoretical framework underlying the approach of this work is based upon the State of Health in the EU, a two-year initiative undertaken by the European Commission and deve...

Patient empowerment of people living with rare diseases. Its contribution to sustainable and resilient healthcare systems.
Authors: De Santis M, Hervas C, Weinman A, Bosi G, Bottarelli V Abstract The notion of empowerment is linked to patients' everyday life and is the base allowing for the patient engagement through which individuals and communities are able to express their needs, are involved in decision-making, take action to meet those needs. In the field of rare diseases, empowerment strategies have greater value due to low prevalence, lack of expertise, poor quality of life. Avenues to patient empowerment are: health literacy and capacity-building; shared decision-making; support to self-management. Patient empowerment is recogn...

Resilience in rare disease networks.
Authors: Jessop E Abstract A resilient system is one which continues to perform its function or goal during a period of change. The original concept was formed around return to the status quo after major external shocks. Recently the concept of "everyday resilience" has been proposed. Everyday resilience is the adaptive and learning response of systems to the daily disturbance of normal routines. For everyday resilience, human factors are as important as physical resources. Rare disease networks can use several strategies to build resilience. PMID: 31553325 [PubMed - in process] (Source: Annali dell I...

Policies and actions to tackle rare diseases at European level.
Authors: Montserrat A, Taruscio D Abstract Rare diseases (RDs) are defined by the European Union as life-threatening or chronically debilitating conditions, with a prevalence lower than 5 per 10 000 inhabitants. Around 6000 diseases are described, affecting between 6% and 8% of the European population. Due to their severity, diffusion and multi-faceted aspects, RD are an area where collaboration in public health, health care and research provides a major integrated added value. Main areas for policy actions include: the development and implementation of European Reference Networks, as a main strategy for sharing of...

Authors: Petrini C Abstract The Law of 3 January 2018 established in Italy the "National Centre for the Coordination of Regional Ethics Committees for Clinical Trials of Medicines and Medical Devices for Human Use" and reduced the number of ethics committees in the country to 40. This Act should, amongst other things, facilitate Italy's compliance with the provisions set forth in European Regulation (EU) 536/2014. Hopefully, in addition to the provisions set forth in the Law, the National Centre will strive to foster the harmonisation of ethics committee procedures, in order to reform Italian legislation ...

Fresh produce and microbial contamination: persistence during the shelf life and efficacy of domestic washing methods.
Authors: Delibato E, Luzzi I, Pucci E, Proroga YTR, Capuano F, De Medici D Abstract The transmission of enteric pathogens by fresh produce depends on the survival of the bacteria organisms during the product shelf-life. The removal of any potentially hazardous microorganism from the vegetables is therefore dependent on the washing and sanitizing techniques employed by individual households. For this purpose, in this work we investigated the persistence of enteric bacteria, using as model Salmonella enterica serovar Napoli (S. Clinical ethics and the role of clinical ethics committees: proposals for a revival. Commentary.
Authors: Petrini C, Ricciardi W Abstract The issue addressed in the paper published by the Italian National Bioethics Committee (NBC) entitled "Clinical ethics committees", is highly significant for many reasons. One of these is the fact that the ethics committees charged with assessing clinical trials have so much responsibility and such a heavy work-load that they have little time available for other tasks such as engaging directly with patients "at the bedside", as a result of which the role of committees responsible for assessing clinical cases is especially important. According to the NBC, ...
