Synchronous tracheostomy and gastrostomy placement results in shorter length of stay in traumatic brain injury patients
American Society for Parenteral and Enteral Nutrition (ASPEN) guidelines recommend gastrostomy for patients suspected to require enteral access device for 4 –6 weeks. Our hypothesis was that traumatic brain injury (TBI) patients undergoing synchronous tracheostomy/gastrostomy (SYNC) compared to tracheostomy first (DELAY) have shorter length of stay (LOS) but higher rates of unnecessary gastrostomy. Given that colorectal cancer (CRC) is the third most commonly diagnosed malignancy in the United States, it comes as no surprise that a significant number of patients who present with an emergency general surgical (EGS) condition have a history of CRC.1 In fact, an analysis of over 3 million EGS presentations highlighted that 8.9  % of such patients have a concurrent malignancy.2 In some of these patients, the history of CRC may be remote and have little or no impact on their current presentation, diagnosis and treatment. Every minute counts
The PROPPR (Pragmatic, Randomized Optimal Platelets and Plasma Ratios) study evaluated the impact of different plasma: platelet: red blood cell ratios at 12 North American academic trauma centers.1 The study prospectively captured times to activation of massive transfusion protocols (MTP) as well as their time to delivery to the patient's bedside. The median time from patient arrival to MTP activation was 9 minutes, with a median time from MTP activation to delivery of the first cooler of 8 minutes. (Source: American Journal of Surgery)
