Waldenstr öm macroglobulinemia: 2017 update on diagnosis, risk stratification, and management
Disease Overview: Waldenström macroglobulinemia (WM) is a lymphoplasmacytic lymphoma with immunoglobulin M (IgM) monoclonal protein. Clinical features include anemia, thrombocytopenia, hepatosplenomegaly, lymphadenopathy, and rarely hyperviscosity. Diagnosis: Presence of IgM monoclonal protein associated with ≥10% clonal lymphoplasmacytic cells in bone marrow confirms the diagnosis. The L265P mutation in MYD88 is detectable in more than 90% of patients. Risk Stratification: Age, hemoglobin level, platelet count, β2 microglobulin, and monoclonal IgM concentrations are characteristics required for prognosis. Ris...

Daily pain in adults with sickle cell disease —a different perspective
Abstract Previous reports demonstrated that patients with sickle cell disease (SCD) experience pain on more than half of the observed days. Yet, these high incidences do not seem to match observations in our population. In this prospective cohort study, we aimed to assess the frequency and characteristics of daily, self‐reported pain among adult SCD patients in the Netherlands. Consecutive patients were enrolled during routine outpatient visits and followed up to 6 months. A total of 55 patients completed 5,982 diary observation days. Median age was 27 years (IQR 23‐43). Patients reported SCD related pain on ...

The Combination of Cyclosporine and Mycophenolate Mofetil is Less Effective than Cyclosporine and Methotrexate in the Prevention of Acute Graft ‐versus Host Disease after Stem‐cell Transplantation from Unrelated Donors
Abstract Acute graft‐versus‐host disease (GVHD) is the major treatment‐related complication after stem‐cell transplantation (SCT) from unrelated‐donors. Several GVHD prophylaxis regimens have been explored, but no regimen has shown superiority. We analyzed transplantation outcomes in 472 consecutive unrelated‐donor SCT recipients, using cyclosporine/methotrexate (MTX, n=314) or cyclosporine/mycophenolate‐mofetil (MMF, n=158) for GVHD prophylaxis. Neutrophil engraftment was faster after MMF, day 11 and 14, respectively (P=0.001). Acute GVHD grade II‐IV and III‐IV occurred in 47% and 28% after MMF compared ...

Increased healthcare use up to 10 years among relapse ‐free Hodgkin lymphoma survivors in the era of intensified chemotherapy and limited radiotherapy
This study aims to provide a global assessment of late adverse effects for working‐age HL survivors treated with contemporary protocols (combination chemotherapy and limited radiotherapy). From Swedish nationwide registers we identified 1017 HL survivors diagnosed in 2000‐2009, aged 18‐60 years (median 32) and surviving at least one year post‐diagnosis, and 4031 age‐, sex‐ and calendar‐year‐matched population comparators. Incidence rate ratios (IRR) and 95% confidence intervals (95%CI) for outpatient visits and inpatient bed‐days after the first year up to 14 years post‐diagnosis (through 2013) were est...

Bone Marrow Cellularity at Day 14 Is the Most Important Predictive Factor for Response in Patients With AML Who Require Double ‐Induction Chemotherapy: Analysis From a Large, Single Institution Experience
Abstract In patients with acute myeloid leukemia (AML), the presence of residual disease at day 14 after primary induction therapy warrants consideration of a second induction cycle. However, data to guide retreatment decisions in such patients are presently limited. Here, we retrospectively reviewed data from 176 patients with AML treated at our institution with a second induction chemotherapy regimen because of day 14 residual disease. Clinical variables and nadir bone marrow features were assessed for correlations with complete remission (CR) and overall survival (OS). In our patient group, 59% achieved CR after a secon...

Polycythemia vera and essential thrombocythemia: 2017 update on diagnosis, risk ‐stratification, and management
Abstract Disease overviewPolycythemia Vera (PV) and essential thrombocythemia (ET) are myeloproliferative neoplasms respectively characterized by erythrocytosis and thrombocytosis; other disease features include leukocytosis, splenomegaly, thrombosis, bleeding, microcirculatory symptoms, pruritus, and risk of leukemic or fibrotic transformation. DiagnosisPV is defined by a JAK2 mutation, whose absence, combined with normal or increased serum erythropoietin level, makes the diagnosis unlikely. JAK2, CALR, and MPL mutations are the mutually exclusive “driver” mutations in ET with respective incidences of 55%, 25%...

The use of biomarkers to describe plasma ‐, red cell‐, and blood volume from a simple blood test
Plasma volume and red cell mass are key health markers used to monitor numerous disease states, such as heart failure, kidney disease, or sepsis. Nevertheless, there is currently no practically applicable method to easily measure absolute plasma or red cell volumes in a clinical setting. Here, a novel marker for plasma volume and red cell mass was developed through analysis of the observed variability caused by plasma volume shifts in common biochemical measures, selected based on their propensity to present with low variations over time. Once a month for 6 months, serum and whole blood samples were collected from 33 activ...

Impact of total body irradiation on successful neutrophil engraftment in unrelated bone marrow or cord blood transplantation
Abstract Total body irradiation (TBI) has been thought to promote donor cell engraftment in allogeneic hematopoietic cell transplantation (HCT) from alternative donors. However, recent progress in HCT strategies may affect the clinical significance of TBI on neutrophil engraftment. With the use of a Japanese transplant registry database, we analyzed 3933 adult recipients (>15 y.o.) who underwent HCT between 2006 and 2013 from an 8/8 HLA‐matched unrelated bone marrow donor (MUD, n=1367), an HLA‐mismatched unrelated bone marrow donor (MMUD, n=1102), or unrelated cord blood (CBT, n=1464). Conditioning regimens were div...
