High-level dolutegravir resistance can emerge rapidly from few variants and spread by recombination: implications for integrase strand transfer inhibitor salvage therapy
The integrase strand transfer inhibitor (INSTI) dolutegravir is commonly used in combination antiretroviral therapy regimens and retains strong potency even with primary resistance mutations to some other INSTIs. Acquisition of accessory mutations to primary mutations results in significant increases in dolutegravir resistance. Previously, we reported that addition of the secondary mutation T97A can result in rapid treatment failure in individuals with INSTI mutations at positions 140 and 148. Here, we conducted a detailed case study of one of these individuals and find that T97A-containing HIV emerged from a large replica...

Associations between HIV viremia during antiretroviral therapy and cardiovascular disease
Objective: To investigate the association between HIV viremia exposure during antiretroviral therapy (ART) and cardiovascular disease (CVD) risk. Design: Nationwide observational cohort. Methods: Participants (age>15 years) from the Swedish nationwide InfCareHIV register initiating ART 1996–2017 were categorized in a time-updated manner into four viremia categories, starting from 12 months after ART initiation: suppression ( (Source: AIDS)

Immune recovery-related patterns of post kala-azar dermal and ocular leishmaniasis in people living with HIV
Conclusion: In PLHIV, PKDL occurs after a cured episode of visceral leishmaniasis as part of an immune restoration disease under ART, or concomitant to a visceral leishmaniasis relapse in a context of AIDS. For the latter, the denomination ‘disseminated cutaneous lesions associated with visceral leishmaniasis’ seems more accurate than PKDL. (Source: AIDS)

Trends in diabetes incidence and associated risk factors among people with HIV in the current treatment era
Conclusions: Rapidly increasing incident diabetes rates among PWH were associated with both traditional and HIV-related associated risk factors, particularly body weight, steroid exposure, and use of Integrase Inhibitors. Notably, several of the risk factors identified are modifiable and can be targeted for intervention. (Source: AIDS)

Trans women have worse cardiovascular biomarker profiles than cisgender men independent of hormone use and HIV serostatus
Background: Feminizing hormonal therapy (FHT) and HIV potentially alter cardiovascular disease (CVD) risk in transgender women (TW). Methods: TW were enrolled in Los Angeles, California and Houston, Texas and frequency-matched to Multicenter AIDS Cohort Study cisgender men (CM) on age, race, substance use, and abacavir use. Biomarkers of CVD risk and inflammation were assessed via ELISA. Wilcoxon rank sum and Fisher's exact tests compared TW and CM. Multivariable linear regression assessed factors associated with biomarker concentrations. Results: TW (HIV+ n = 75, HIV− n = 47) and CM (HIV+ n =...

Durability of switched therapy after failure of WHO-recommended antiretroviral therapy regimens in a resource-limited setting
Conclusion: Switched therapies are durable for 8 years after failure of recommended regimens. A high preswitch viral load, ATV/r-based regimens, and young adulthood are risk factors for viral rebound, which underscores the need for more durable regimens and differentiated care services. The effect of an HIV preexposure prophylaxis panel management strategy to increase preexposure prophylaxis prescriptions
Conclusions: A panel management intervention using PrEP coordinators and a web-based panel management tool increased PrEP prescribing and improved PrEP-related counseling in safety-net primary care clinics. (Source: AIDS)

Maternal–foetal transfer of severe acute respiratory syndrome coronavirus 2 antibodies among women with and those without HIV infection
In pregnant women, antibodies against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike protein cross the placenta and can be detected in cord-blood at the time of delivery. We measured SARS-CoV-2 full-length antispike IgG in blood samples collected from women living with HIV (WLWHIV) and without HIV when presenting for labour, and from paired cord-blood samples. Antispike IgG was measured in maternal blood at delivery on the Luminex platform. Cord-blood samples from newborns of women in with detectable antispike IgG were analysed. The IgG geometric mean concentrations (GMCs) and the percentage of cord-blo...

Intersectional stigmas are associated with lower viral suppression rates and antiretroviral therapy adherence among women living with HIV
Conclusion: The negative impact of intersectional stigmas on viral suppression is likely mediated, but not fully explained, by reduced ART adherence. We discuss the research and clinical implications of our findings. (Source: AIDS)

Qualitative plasma viral load determination as a tool for screening of viral reservoir size in PWH
Conclusion: Our study shows that PTD provides an efficient parameter to preselect participants with a small viral reservoir based on already available pVL data for future HIV cure trials. (Source: AIDS)

Intersecting epidemics: the impact of coronavirus disease 2019 on the HIV prevention and care continua in the United States
Conclusion: Although heightened disparities for people at risk for, and living with, HIV were seen during the COVID-19 pandemic, many health systems and clinics have achieved and maintained engagement in HIV prevention and care. This review highlights barriers and innovative solutions that can support durable and accessible health systems through future public health crises. (Source: AIDS)
