Patients' Illness Perception as a Tool to Improve Individual Disease Management in Primary Cutaneous Lymphomas.
This study investigated the clinical relevance of the relationships between illness perception, illness behaviour, and HRQOL in CTCL and cutaneous B-cell lymphomas (CBCL). A total of 92 patients completed the IPQ-R, the Scale for the Assessment of Illness Behavior (SAIB), and a skin-specific HRQOL tool (Skindex-29). Data on illness behaviour were not evidently related to illness perception, whereas illness perception was significantly associated with HRQOL. Both, IPQ-R and HRQOL results correlated with disease entity, stage, and socio-demographics. Only IPQ-R results provided practical information on patients' needs to tra...

Highlighting Interleukin-36 Signalling in Plaque Psoriasis and Pustular Psoriasis.
Abstract Plaque psoriasis and pustular psoriasis are overlapping, but distinct, disorders. The therapeutic response to biologics supports the pivotal role of the tumour necrosis alpha (TNF-α)/ interleukin (IL)-23/IL-17/IL-22 axis in the pathogenesis of these disorders. Recently, functional activation of the IL-36 receptor (IL-36R) was discovered to be another driving force in the pathogenesis of psoriasis. This was first highlighted by the discovery that a loss-of-function mutation of the IL-36R antagonist (IL-36Ra) causes pustular psoriasis. Although the TNF-α/IL-23/IL-17/IL-22 axis and the functional...

Topical Glucose Induces Claudin-1 and Filaggrin Expression in a Mouse Model of Atopic Dermatitis and in Keratinocyte Culture, Exerting Anti-inflammatory Effects by Repairing Skin Barrier Function.
This study examined the effect of topical application of high-dose glucose on inflamed skin in a murine model of AD. High-dose glucose treatment on the ear reduced dermatitis scores and ear thicknesses in mite antigen-treated NC/Nga mice. The levels of thymus and activation-regulated chemokine (TARC), Th cytokines (interleukin (IL)-4, IL-5, IL-12, IL-13, and (interferon) IFN-γ), and IgE were decreased in the serum of high-dose glucose-treated mice. Expression of claudin-1 and filaggrin was reduced in the ear epithelium in the NC/Nga mice. However, the reduced expression was restored by topical treatment with high-dos...

Role of Substance P and Its Receptor Neurokinin 1 in Chronic Prurigo: a Randomized, Proof-of-Concept, Controlled Trial with Topical Aprepitant.
Abstract Substance P (SP) and its receptor neurokinin 1 (NK1R) are thought to be involved in the pathogenesis of chronic prurigo. Here, we assessed SP serum levels, cutaneous NK1R expression, and the effects of topical aprepitant, an NK1R antagonist, in patients with chronic prurigo. SP and NK1R were increased, compared with controls, in the serum and in lesional vs. non-lesional skin of the patients, respectively. Aprepitant, in a randomized, placebo-controlled, split-sided, double-blind trial, reduced the intensity of pruritus as assessed by visual analogue scale by> 50% from baseline to day 28 (-35.2)...

Incidence and Regression of Actinic Keratoses in Organ Transplant Recipients.
Abstract Actinic keratoses (AKs) are highly dynamic lesions and AK activity has been shown to be associated with squamous cell carcinoma (SCC). We sought to explore risk factors which may affect the 12-month turnover of AKs in organ transplant recipients (OTRs). The number of incident AKs, regressed AKs and net change in AK counts were calculated. Negative binomial regression and Poisson regression models were used to estimate rate ratios (RR) for these 3 outcomes. Among 150 renal and 89 liver OTRs, those who spent> 50% of a typical weekday in the sun had a lower rate of AK regression than those who spen...

Temporal Stability of the Healthy Human Skin Microbiome Following Dead Sea Climatotherapy.
Abstract Dead Sea climatotherapy (DSC) is a therapeutic modality for a variety of chronic skin conditions, yet there has been scarce research on the relationship between the cutaneous microbiota and disease states in response to DSC. We characterized the skin bacterial and fungal microbiome of healthy volunteers who underwent DSC. Bacterial community diversity remained similar before and after treatment, while fungal diversity was significantly reduced as a result of the treatment. Individuals showed greater inter-individual than temporal bacterial community variance, yet the opposite was true for fungal community...
