Office Fluoro Contrast Billing
For those of you who do in office fluoro - how do you handle billing for contrast? Our EMR (ModMed) calculates billing meds by mL used and so 1 mL of Omnipaque-300 is billed ~$0.40. Which is wayyy below cost of Omnipaque. Does everyone just eat the cost of contrast at a loss or am I missing something with how contrast should be billed? Our billing team wants to bill by 'unit' and argues 1 mL = 300 units of Omnipaque-300 - this seems incorrect at welcoming an audit nightmare. Any... Read more (Source: Student Doctor Network)

omnipaque concentration for procedures
i've been using omnipaque 300 or 240 for procedures. currently those are out of stock, but i can get 180. any downside to using 180? from what i gather 140 and 350 can't be used intrathecal, but rest are ok. i'm assuming visualization is also ok with other concentrations? Omnipaque 2.0
Thought I would share our current Omnipaque Strategy: Hospital pharmacy takes a 50 ml vial and divides it into 25 2 ml doses in separate syringes. This is done under their hood (they already do this with certain specialty meds) These doses are packaged, dated together in a dated bag, and refrigerated for a 10 day life span when my day surgery team tees things up they pull one dose per pt that a.m.. If I need more than 2ml they can borrow from a later case and grab a replacement from the... Read more (Source: Student Doctor Network)

Contrast questions
Hope all is well. Had a few questions I could not find definitive answers to (probably because there are none) but was hoping for some guidance. 1.) Do some of you use Gadolinium for ESI's (CESI, TFESI, LESI) etc? If so what are your thoughts on case reports of inadvertent intrathecal injection and death? 2.) If someone has anaphylaxis to shellfish are you avoiding omnipaque or are you using a steroid prep? Thanks! (Source: Student Doctor Network)
