SCIP guidelines
Have a handful of GYN surgeons who outright refuse all antibiotics for diagnostic laparoscopies, ectopic pregnancies ; just iodine over the belly and go. Digging around in the pelvis, mobilizing and cutting tissues for hour or two without antibiotics seems bizarre to me. Any laparoscopic surgeries by gen surg include preincision antibiotics as per SCIP. Where is the line if we feel it is clearly indicated and the surgeon outright refuses other then to proceed and document their refusal... SCIP guidelines (Source: Student Doctor Network)

Gadolinium fatality
I have been using in patients with iodine allergies for ESI/TFESI See the attached case report. They inadvertently injected intrathecal during MILD, and the patient died. Amazingly, it was only 5cc of contrast. What are your thoughts? (Source: Student Doctor Network)

Thyroid Neck Recurrence
I got a guy that presented 2 years ago with metastatic thyroid cancer; they did a hemithyroidectomy at presentation to get more definitive diagnosis. Turned out to be hurthle cell ca. They did adjuvant RAI after this although the tumor wasn't hot for iodine uptake. There are 4 mets in the lungs that is it, they are about 1-2 cm and one is 2-3 cm. The primary grew back in the neck and is about 3-4 cm and up against the esophagus and not far from the right Bplexus. They could give systemics,...
