Impact of palliative radiotherapy with or without lung irradiation in patients with interstitial lung disease
CONCLUSION: High PFS and lung-RT are significant risk factors for AE-ALI/RP grade ≥3. Even with relatively low doses, palliative-intent lung-RT carries an extremely high risk of AE-ALI/RP when PFS is high.PMID:36963439 | DOI:10.1016/j.radonc.2023.109636 (Source: Radiotherapy and Oncology : journal of the European Society for Therapeutic Radiology and Oncology)

Post-COVID interstitial lung disease in symptomatic patients after COVID-19 disease
AbstractCOVID-19 is often associated with long-lasting pulmonary symptoms. Data are scarce about interstitial lung disease (ILD) in patients following COVID-19 hospitalization with persistent symptoms. We retrospectively reviewed all cases sent to pulmonary post-COVID evaluation due to persistent symptoms between February 2021 and February 2022 (N  = 318). All patients with suspected ILD (N = 44) were reviewed at the multidisciplinary discussion. Patient characteristics, symptoms, time since hospitalization, detailed lung function measurements and 6-min walk test (6MWT) were evaluated. The post-COVID ILD suspected ...

Practicable diagnostics of Sj ögren's syndrome in interstitial lung disease-A discussion article
Z Rheumatol. 2023 Mar 22. doi: 10.1007/s00393-023-01344-3. Online ahead of print.ABSTRACTSjögren's syndrome (SjS) is a possible autoimmune cause of interstitial lung disease. The diagnostic pathway for SjS, however, is largely undefined in comparison to other systemic autoimmune diseases. Subjective sicca symptoms, anti-SS-A/Ro antibodies and even ANA as screening tests all have relevant limitations in sensitivity and/or specificity. Against this background, in an interdisciplinary discussion we have developed a consensus for the clarification of SjS, which is presented here for broader discussion. In addition to ANA and ...

The IL-33:ST2 axis is unlikely to play a central fibrogenic role in idiopathic pulmonary fibrosis
CONCLUSIONS: Together these findings suggest the IL-33:ST2 axis does not play a central fibrogenic role in the lungs with therapeutic blockade of this pathway unlikely to surpass the current standard of care for IPF.PMID:36949463 | DOI:10.1186/s12931-023-02334-4 (Source: Respiratory Care)

Virtual communication is commonly used in Finnish interstitial lung disease multidisciplinary meetings
Eur Clin Respir J. 2023 Mar 17;10(1):2190210. doi: 10.1080/20018525.2023.2190210. eCollection 2023.ABSTRACTMultidisciplinary meeting (MDM) is a core element in the diagnosis of interstitial lung diseases (ILD). The aim of the study was to investigate the implementation and key elements related to ILD MDMs in Finnish specialized care, which is characterized by long travel distances and a large number of small centers treating patients suffering from ILDs. An electronic questionnaire was sent to ILD experts working at five academic centers of Finland regarding the implementation of ILD MDMs with the focus on utilization of v...

Increasing dyspnoea and intermittent joint suffering
Pneumologie. 2023 Mar 23. doi: 10.1055/a-2016-7215. Online ahead of print.ABSTRACTIn rheumatic diseases the possibility of pulmonary manifestation must always be considered and checked. Interstitial lung disease can often be fatal in these cases. In the presented case, the link between progressive dyspnea and newly occurring skin irritation is to be seen as particularly important. A good outcome can be achieved by immediate therapy with immunosuppression and plasmapheresis.PMID:36958340 | DOI:10.1055/a-2016-7215 (Source: Pneumologie)

Upper respiratory tract mycobiome alterations in different kinds of pulmonary disease
In this study, we investigated the fungal community composition of oropharynx swab (OS) samples from patients with five kinds of pulmonary disease, including interstitial lung disease (ILD), bacterial pneumonia (BP), fungal pneumonia (FP), asthma (AS) and lung cancer (LC), and compared them with healthy controls (HCs), based on high-throughput sequencing of the amplified fungal internal transcribed spacer (ITS) region.ResultsThe results showed significant differences in fungal composition and abundance between disease groups and HCs. Malassezia was the most significant genus, which was much more abundant in pulmonary disea...

Successful baricitinib treatment of refractory anti ‐synthetase syndrome associated with interstitial lung disease
We present a case of refractory ASS-ILD successfully treated with baricitinib after the failure of several other treatments. Bariticinib shows promise as a salvage treatment for patients with refractory ASS-ILD. AbstractA 47-year-old Japanese man was admitted with dyspnoea on exertion (DOE), skin rash and myalgia. Clinical findings of Gottron's sign and mechanic's hands were observed, with increased serum levels of Krebs von den Lungen-6, surfactant protein-D, creatine kinase, and anti-EJ on laboratory tests. In both lungs, chest computed tomography revealed diffuse reticular opacities and lower lobe predominance. The pati...
