NonInvasive Vagus Nerve Stimulation in Spontaneous Subarachnoid Hemorrhage (VANQUISH): a Randomized Safety and Feasibility Study
Spontaneous subarachnoid hemorrhage (SAH) typically presents with severe thunderclap headache, often described as the “worst headache of life”. The vast majority of patients (90%) have persistent severe headache while in the intensive care unit [1] and more than a third continue to have this headache for years following cerebral insult, which adversely affects their quality of life [2]. Currently, there is lack of effective treatments or guidelines, and the data evaluating the efficacy of analgesic medications for these patients is scarce. Exploring 6  years of colorectal cancer surgery in rural Italy: insights from 648 consecutive patients unveiling successes and challenges
AbstractThe multidisciplinary management of patients suffering from colorectal cancer (CRC) has significantly increased survival over the decades and surgery remains the only potentially curative option for it. However, despite the implementation of minimally invasive surgery and ERAS pathway, the overall morbidity and mortality remain quite high, especially in rural populations because of urban  − rural disparities. The aim of the study is to analyze the characteristics and the surgical outcomes of a series of unselected CRC patients residing in two similar rural areas in Italy. A total of 648 consecutive patients o...

Acute Heart Valve Emergencies
Within the cardiac intensive care unit, prompt recognition of severe acute valvular lesions is essential because hemodynamic collapse can occur rapidly, especially when cardiac chambers have not had time for compensatory remodeling. Within this context, optimal medical management, considerations for temporary mechanical circulatory support and decisive treatments strategies are addressed. Fundamental concepts include an appreciation for how sudden changes in flow and pressure gradients between cardiac chambers can impact hemodynamic and echocardiographic findings differently compared to similarly severe chronic lesions, as...

Nutrition research in the ICU
Critical care management of patients' and families' myriad of needs both within, and beyond intensive care units (ICUs) requires a comprehensive, multidisciplinary evidence-based approach. As demonstrated by the breadth of papers in the current Australian Critical Care (ACC) issue, patients in ICU are critically ill across the lifespan and developing evidence to mitigate risk demands dynamic, multiple methods of inquiry. In the last ACC issue, Marshall (2024) asked “Are we ready?” for the expected increased average age of patients admitted to the ICU compared to today's cohort. (Source: Australian Critical Care)

Partial versus Complete Sternotomy for Aortic Valve Replacement-Multicenter Study
CONCLUSION: In a large, German multicenter cohort, MACCE rates were comparable in surgical aortic valve replacement through partial upper and complete sternotomies. Shorter ICU stay and lower rates of Dressler's syndrome and rehospitalization were in favor of the partial sternotomy group.PMID:38626902 | DOI:10.1055/s-0044-1782685 (Source: The Thoracic and Cardiovascular Surgeon)

CONCLUSION: The surgical ascending aorta replacement through a partial sternotomy is associated with longer operative times, but this does not affect the early as well as the long-term follow-up.PMID:38626901 | DOI:10.1055/s-0044-1782186 (Source: The Thoracic and Cardiovascular Surgeon)
