Cash pay for medicaid patients who's injections are denied
Does anyone know if it is a breach of your medicaid contract to offer patients a cash pay option for an injection such as a LESI or TFESI if medicaid denies the prior-authorization? For example, I just had Meridian deny a 2 level TFESI on a patient because he had a prior epidural done 8 months ago by a different doctor that didn't help. Medicaid insurance and new solo private practice
Hello! I am in the process of opening a solo psychiatric practice this summer and currently getting credentialed with some of the commercial insurers and Medicare. During this credentialing process, a blue cross blue shield agent said, "If you choose not to accept Medicaid Managed Care members, this would be for all health plans, not just for BC/BS. Insurance Denial of IMRT for Prostate
Anyone here run into this before? I've got a P2P tomorrow with Priority Health. They're denying IMRT for 28-fraction prostate treatment for an intemediate risk prostate cancer. They're telling me they'll only approve brachy or 3D. This is the first time I've ever had IMRT denied for prostate. It's standard of care and am frankly shocked by the denial. Person I'm talking to tomorrow is not a rad onc. They don't have a rad onc to talk to. Are other folks seeing this kind of shenanigans? (Source: Student Doctor Network)

Independent Contractor vs. Employee (1099 vs. W2) in Podiatry
Hi everybody, A resident asked me a question and I thought I would relay it here since you all seem much smarter than me. From our perspective, what are the advantages of working as an independent contractor instead of just being a W2 employee as a Podiatry associate or for one of those other groups (Upperline, etc.)? Disability insurance $900/month
Guardian is quoting $900/month for $8000 coverage. Mass mutual is quoting $750/month. Both seem very expensive. Do I go for it or leave it? I don't have any medical issues, I don't know the future and if buying the insurance will be worth it. Any advice? (Source: Student Doctor Network)

Cancer in medical school
Hello I'm writing this in hope that who ever may be in my situation be inspired to stay strong. A bit of background for you, I'm a healthy 28 YO with no chronic health issues. Household spending and inflation 2022
Thought it would be interesting to see what other anesthesiologists are spending. I’ll share first. We spent $120k in 2021. Inflation hit plus we spent 15k on my man cave and had some annoying house expenses like tree work. $140k in 2022. House is paid off so that’s saves us 20-30k. And heath insurance is covered by work. Do I have to disclose lattice degeneration in disability insurance questionnaire?
I'm currently filling out the medical questionnaire for a MassMutual disability insurance, and one of the questions asks if I have been treated for, diagnosed with, or advised about the following: "A disorder of the eyes, ears, nose, throat or sinuses such as any partial or complete loss of hearing, vision or speech, acoustic neuroma and optic neuritis?" EM to Pain

Getting on with insurance panels
Ok, I will admit this is an area I could use some assistance in. Back in Ohio in my former LLC I attempted to get on with United Healthcare I believe and their website basically outlined I needed to first go through CAQH. I did that. Payment for 64999 nerve blocks (IPACK, PENG, Erector Spinae)
How are your groups handling insurance (CMS included) denials for payments for newer nerve blocks. We have transitioned to IPACK, PENG, and erector spinae blocks for a vast majority of our patients. Per the AMA instructions, they are coded as 64999, but we are still frequently are not paid for them. They often are rebuked by saying it is "experimental." Any guidance would be appreciated.
