A Case of an Obstructed Delivery by a Large, Lower Uterine Segment Fibroid Interlocked with a Fetal Mentum
We describe a case of an obstructed delivery by a large fibroid. A 37-year-old G2P1001 with a 10-cm anterior, lower uterine segment fibroid presented for labor induction. Labor was complicated by arrest of descent due to suspected obstruction of the fetal body by the fibroid after descent of the fetal head, and delivery during cesarean section was complicated by apparent interlocking of the fetal mentum with the fibroid. A prospective randomized double-blind study comparing the dose-response curves of epidural ropivacaine for labor analgesia initiation between parturients with and without obesity
Conclusion: In conclusion, within the parameters of this study, our findings suggest that obesity, characterized by a mean BMI value of 30.9, does not significantly influence the EC50 and EC90 values of epidural ropivacaine for labor analgesia. Methods of induction of labor in women with obesity: A secondary analysis of two multicenter randomized controlled trials
ConclusionsIn women with obesity we found a nonsignificant trend towards an increased rate of cesarean sections in the group induced with a Foley catheter compared to oral misoprostol; however, the study lacked power for this subgroup analysis. Labor induction at 41+0 gestational weeks or expectant management for the nulliparous woman: The Finnish randomized controlled multicenter trial
ConclusionsOffering labor induction to nulliparous women at 41+0 gestational weeks may decrease the probability of operative delivery, postpartum hemorrhage, and neonatal weight ≥4000 g. However, this study was underpowered to affirm the trends of rising rates of CS and adverse neonatal outcomes in the expectant management group. Optimal Misoprostol Dosing Among Patients with a BMI Greater Than 30: A Randomized Controlled Trial
Patients with obesity experience an increased duration of labor with increased risk of perinatal morbidity. Compared to parturients without obesity, they also experience fewer uterine contractions after administration of misoprostol. It is unclear whether the same dose of misoprostol should be used for induction of labor in patients with obesity compared to non-obese patients. Disruption of the SP-A/SP-R210 < sub > L < /sub > (Myo18A α) pathway prolongs gestation and reduces fetal survival during lipopolysaccharide-induced parturition in late gestation
We report that either lack of SP-A or disruption of SP-R210L delays parturition by 0.40 and 0.55 days compared to controls, respectively. LPS induced labor 0.60, 1.01, 0.40, 1.00, and 1.31 days earlier than PBS controls in WT, SP-A-deficient, littermate controls, heterozygous, and homozygous SP-R210L-deficient mice, respectively.
