Pulmonary embolism (PE) is a common and potentially fatal diagnosis that must be considered in the emergency department (ED) setting in patients presenting with chest pain or shortness of breath. PE accounts for approximately 100,000 deaths annually in the United States and, according to some studies, this rate is increasing (1). Although computed tomography pulmonary angiography (CTPA) is the gold standard imaging modality for the diagnosis of PE, point-of-care ultrasound (POCUS) is often more readily available and can be used to quickly obtain critical information regarding global cardiac function and direct management.

Awareness of this uncommon manifestation would help differential diagnosis of periostitis and could help decrease the patient's anxiety. It should also be noted that periosteal reactions by benign entities could cause aggressive-looking periosteal reactions in post-operative regions.

A 35-year old man, with a deep pectus excavatum due to a Marfan syndrome treated 9 years before for an acute type A dissection involving only the aortic arch, by a Bentall surgery, was admitted for acute chest pain. CT scan showed an Acute type NA-NB dissection extending to the iliac. After 5 days with strict arterial blood pressure management, the patient had recurrent refractory chest pain and an hybrid technique associating full supra aortic vessels debranching and STABILISE technique during the same procedure was performed.

CONCLUSIONS: A minimally invasive total endovascular approach allows, through local anesthesia and percutaneous access, to avoid surgical cut down and orotracheal intubation. This, combined with a defined management protocol for infected patients, seems to be a reasonable way to perform endovascular aortic procedures in urgent setting, even in a SARSCoV- 2 positive patient.

Conclusion: We believe this rare case of pulmonary invasive schwannoma illustrates the complex dynamics of this extremely rare entity; in this particular case, complete surgical excision proved to be crucial in terms of subacute management and local tumor control, at least at short and middle term. We present a video of the laparoscopic management of an IH that occurred after an OAGB. The patient was a 49-year-old female who had undergone a laparoscopic revisional OAGB 2 years previously after a failed laparoscopic adjustable gastric banding.
