Cauda Equina in elderly with severe stenosis
Did an SIJ on an 86 yo patient last month who also has severe L3-4, L4-5 CCS. Called me a few days later complaining of saddle anesthesia (can't feel toilet paper when wiping) and new right lower extremity weakness. No b/b incontinence. Instructed pt to go to ED for suspected CES likely due to prone positioning. Pt overwhelmed and wanted me to convey to daughter which I did. Don't think they went. Have not seen since. Colorectal ADK in 95 yo patient
Dear colleagues, I have a 95 yo female patient, very well conserved, ambulant . No severe comorbidities. She presented for low rectum ademocarcinoma invading partially the internal anal sphincter with no fecal incontinence. She complains of post defecation pain and rectorrhagia. Pelvic MRI showed no LNs. She is not operable. She will receive xeloda with concomitant radiation. NBME Peds Q really bothering me, IgA Nephropathy vs PSGN
13 year old boy was brought in for a routine well-child exam. No history of prior serious illness. During the past 2 years he's complained of brown urine whenever he's had a cold. He has no complaints of urgency, dysuria, or incontinence. PE is unremarkable.
