Patient being evaluated for liver transplant at a major transplant center. Their MELD score is over 30. They passed all the requirements to be presented to the transplant committee. Although the nuclear stress test was normal and they have no cardiac history, a cardiac cath was performed. It revealed RT heart dominance LAD 60 % diffuse dx with multiple discrete lesions CX 60% distal discrete dx Om2 50% diffuse dx EF nl No cardiac sx ever, even with End Stage Liver Dx. Thoughts on working in a thoracic transplant (evlp) lab for a cardiac resident?
What are people's thought's on doing 3-4 years of evlp work in a thoracic lab and trying to tie that to cardiac transplant if your career goal is to become a cardiac transplant surgeon? Would that be a bad idea. The alternative would be to try to do similar but heart related work in a lab that may potentially be less desirable to work in from a lab environment/supervisor perspective with a weaker track record. Liver Transplant
New CA2. I just finished my first liver transplant and it was awesome. Pt was late 60s hx of chronic Hep C and met colon CA to liver s/p resection 7 yrs ago. Diabetes, HTN but normal heart. Case went great despite lasting 9 hrs and losing 2L of blood since it was a re-operation. "Usual" case length is 4-5 hours. 6 units of PRBCs, 5 units FFP, 6pk platelets, some cryo, 1L albumin, and 2.5L crystalloid. We went upstairs with normal ABG and no pressors.
