Older patients with delusional disorder
Lately ive had quite the influx of older females (>65 years old) with delusional disorder (no other psychotic sx, pure delusions, no obvious medical etiology). These are quite hard to treat as you can imagine, due to low insight, questionable med compliance (i try to get family involved as much as possible) and poor response to antipsychotics vs other psychotic sx. Geriatric Unit Dispositions

Psychiatry Scribe or ED Scribe?
I recently got two scribe jobs, one is in the psychiatry department and the other is in the ED.
The ED one is with a larger scribe company
The psychiatry one is with a smaller, contract company and will allow me to work in multiple psychiatry units (behavioral health, geriatric psychiatry, crisis stabilization, and the ER (limited to psychiatric patients I would think)). My supervisor said they will also be able to provide a letter of rec after my completion.

primary care physician/geriatrician and hospice/palliative care focus
I've volunteered a lot with hospice patients, for nearly five years now. It's extremely meaningful to me and I want to work with the elderly population as a primary care physician, because the aging population faces a lot of complex challenges and I want to help them. Is this okay to have this as my main focus for the "career goals/where do you see yourself in ten years" supplemental essay? I have some other plans but this is my main focus. Is this too narrow?

methylphenidates/stimulants in geriatric depression, as an adjunct medication
Any general thoughts on this? I treat a large pool of geri patients, id say >50% are geri or close to it. A lot of men tend to not do so well with age, become severely depressed, lay in bed all day and have zero motivation. Had one guy in his late 60s who had been on adderall 40 xr BID (no misuse on his part, just an idiot prescriber) and he did not do well when he went off.
