Psychiatry Pharmacology Resources
Hello everyone, I am an incoming PGY-1 psych resident and I wanted to learn about some of the best sources for psychiatry pharmacology. I have the Stahl's prescriber's guide but while it is certainly comprehensive, I feel that I would like a condensed version as well. For example, it would be helpful to learn about the highlights of certain drugs such as "Celexa is one of the SNRIs that prolong the QT the most" or "Haldol and Geodon can prolong the QT the most out of antipsychotics. Starting ziprasidone at 20 mg vs 40 mg
I can't remember the details, but has it been recommended to start ziprasidone at 40 mg BID instead of 20 mg BID? I seem to recall the 20 mg BID was associated with more side effects, like akathisia? (Source: Student Doctor Network)

Pushing antipsychotics beyond maximum doses
I constantly see psychiatrists going above the maximum recommended dose of antipsychotics (not even mentioning max effective dose). Zyprexa 30-40mg, geodon 320mg, high doses of risperdal. It is a personal pet peeve of mine and I especially hate it when they are on multiple antipsychotics along with the excess doses. I always adhere to the max FDA dose even in severe patients in the hospital, do you guys agree? (Source: Student Doctor Network)

What PRN medication would you prescribe for agitation in outpatient setting?
The patient is a young adult with unspecified mental handicap who comes with mom. He has been on the following regimen for years: clonidine 0.3mg per day depakote DR 1000mg per day geodon 200mg per day valium 20mg per day (Most of the above medications are split into two dosings a day.) He has been on the regimen for years and is tolerating medications. Mother does not want to change the treatment.
