Delayed diagnosis of serious paediatric conditions in 13 regional emergency departments
Conclusion Delayed diagnosis was uncommon across 13 regional EDs but was more likely among children with Hispanic ethnicity, CCCs or normal vital signs. Delays were associated with a higher risk of complications. (Source: BMJ Quality and Safety)

Quality framework for remote antenatal care: qualitative study with women, healthcare professionals and system-level stakeholders
Conclusions This study offers a provisional framework that can help in structuring thinking, policy and practice. By outlining the range of domains relevant to remote antenatal care, this framework is likely to be of value in guiding policy, practice and research. (Source: BMJ Quality and Safety)

What and when to debrief: a scoping review examining interprofessional clinical debriefing
Conclusions The proposed categories offer a practical way to examine and discuss CD which may inform decisions about implementation. By differentiating CD according to relevant contextual factors, these categories may reduce confusion which currently hinders discourse and implementation. The findings from this review promote context-specific language and a shift away from conceptions of CD that embody a one-size-fits-all approach. (Source: BMJ Quality and Safety)

Equitable and accessible informed healthcare consent process for people with intellectual disability: a systematic literature review
Conclusions Multiple reasons contribute to poor consent practices for people with intellectual disability in current health systems. Recommendations include addressing health professionals’ attitudes and lack of education in informed consent with clinician training, the co-production of accessible information resources and further inclusive research into informed consent for people with intellectual disability. Interrupting false narratives: applying a racial equity lens to healthcare quality data
Introduction Across the globe, awareness of racial and ethnic health inequities and the need for healthcare systems to address them are growing.1 In many countries, teams working in healthcare institutions are encouraged to stratify quality metrics by race and ethnicity as part of the movement to identify and disrupt health inequities.2 The lack of standard definitions for race and ethnicity and restrictions on how and where these data can be collected make collecting high-quality data challenging. Stratifying quality metrics by race and ethnicity is further challenged by the risk of undermining the core goal of disrupting...

Prophylactic Biosynthetic Retrorectus Mesh Placement During Stoma Reversal Reduces the Rate of Stoma Site Incisional Hernia
The objective of this study was to assess the use of prophylactic biosynthetic mesh at the time of stoma reversal on the overall SSIH rate.METHODS: This is an observational retrospective cohort study. A review of 101 consecutive patients who underwent PMR in the retrorectus plane from 2015 to 2020 was compared to 73 consecutive patients who underwent primary stoma closure without mesh from 2011 to 2014. The primary endpoint was the presence of SSIH on clinical examination or computed tomography after ostomy takedown.RESULTS: In total, 174 cases were analyzed with 101 patients in the treatment group (median follow-up 45.2 m...

Disparities in Maternal Health Visits Between Rural and Urban Communities in the United States, 2016-2018
CONCLUSION: Efforts to ameliorate rural-urban differences in maternal care access and quality should explicitly adopt multilevel, systemic approaches to policy and program implementation and evaluation. Policymakers and practitioners should consider telehealth as a potential complementary tool to minimize gaps in quality of care which disproportionately impact rural-dwelling birthing people.PMID:38650474 | DOI:10.7812/TPP/23.067 (Source: The Permanente journal)

CONCLUSION: Eleven years after the introduction of HPV vaccination, a drop in the incidence of cervical cancer was observable at the population level in the birth cohorts eligible for vaccination.PMID:38652841 | DOI:10.3238/arztebl.m2024.0062 (Source: Deutsches Arzteblatt International)

Conservative Versus Operative Treatment of Proximal Humerus Fractures in Older Individuals —an Analysis of Insurance Data
CONCLUSION: The conservative treatment of proximal humerus fracture is associated with a lower overall rate of complications due to surgery or trauma, but also with more MAE and TE and higher overall mortality. These findings underline the need for individualized and risk-adjusted treatment recommendations.PMID:38652842 | DOI:10.3238/arztebl.m2024.0059 (Source: Deutsches Arzteblatt International)

Modification of velopharyngeal closure pressures during phonation by neuromuscular electrical stimulation in healthy individuals
CONCLUSIONS: NMES in combination with articulation results in a change of the velopharyngeal closure pattern with a pressure increase of around 200% in healthy individuals. This might be of therapeutic benefit for patients with velopharyngeal insufficiency.PMID:38651019 | PMC:PMC11034089 | DOI:10.3205/000329 (Source: GMS German Medical Science)

Differences in velopharyngeal pressures during speech sound production in patients with unilateral cleft lip and palate (UCLP) and healthy individuals
CONCLUSIONS: It can be concluded that velopharyngeal pressures of patients with UCLP are not sufficient to eliminate nasal resonance or turbulence during articulation, especially for more complex sounds. These results support a general understanding of hypernasality during speech implying a (relative) velopharyngeal insufficiency.PMID:38651020 | PMC:PMC11034380 | DOI:10.3205/000328 (Source: GMS German Medical Science)

Detection of significant liver fibrosis in Chinese psoriasis patients receiving methotrexate: a comparison between transient elastography and liver histology
CONCLUSION: Transient elastography is reliable and superior to the TCD for detecting liver fibrosis in Chinese psoriasis patients receiving MTX. Liver biopsy should be reserved for high-risk patients or patients with liver stiffness ≥11.7 kPa on TE.PMID:38651202 | DOI:10.12809/hkmj2210364 (Source: Hong Kong Medical Journal)
