Perspectives in treatment-resistant depression: esketamine and electroconvulsive therapy
Wien Klin Wochenschr. 2024 Apr 17. doi: 10.1007/s00508-024-02358-w. Online ahead of print.ABSTRACTModern electroconvulsive therapy (ECT) and the approval of nasal esketamine for clinical use have significantly improved the approach to treatment-resistant depression (TRD), which is defined as non-response to at least two different courses of antidepressants with verified adherence to treatment, adequate dosage, and duration of treatment. The goal of this literature review is to present the newest evidence regarding efficacy and safety. Furthermore, we aim to provide an overview of future perspectives in this field of resear...

Hepatic micro ‐abscesses as an unusual initial presentation of systemic lupus erythematosus: A case report
We report the case of a 27-year-old female who presented with persistent high-grade fever, bilateral exudative lymphocytic pleural effusion, hepatic micro-abscesses, anemia, and lymphopenia. Despite extensive investigations and antibiotic therapy, the patient's condition continued to worsen. The diagnosis of hepatic vasculitis, a rare manifestation of SLE, was ultimately made based on clinical suspicion, positive autoimmune markers, and negative septic workup. The patient responded well to high-dose corticosteroid therapy and intravenous immunoglobulin, with resolution of liver lesions and clinical improvement. Hepatic inv...

A young patient with Graves' disease presenting with a triad of heart failure, pancytopenia, and jaundice: A case report
We describe a case of a young man who presented with progressive cough, increased abdominal girth and ankle swelling for a few months. On examination, he had jaundice, bilateral exophthalmos, diffuse goiter, ascites, and significant lower limb edema. Laboratory investigations showed increased Thyroxin level with a suppressed thyroid stimulating hormone and positive anti-thyrotropin receptor antibodies. Also, the patient had pancytopenia, coagulopathy and cholestatic pattern of elevated liver enzymes. Echocardiography demonstrated mildly reduced left ventricular function with diastolic dysfunction, but electrocardiogram did...

Be prepared for the unexpected: Mind ‐stretching in ST‐elevation myocardial infarction with situs inversus totalis
Key Clinical MessageSitus inversus totalis is a rare diagnosis, but the likelihood of experiencing myocardial infarction is presumed to be comparable to the general population average. In individuals exhibiting situs inversus with suspected myocardial infarction, ECG recording, including right precordial leads, is crucial for diagnostic assessment. Coronary angiography and intervention should be performed with standard equipment using inverted maneuvers and radiographic projections. (Source: Clinical Case Reports)

Simultaneous occurrence of transient global amnesia and Takotsubo syndrome triggered by caring for a terminally ill relative
Key Clinical MessageTakotsubo syndrome and transient global amnesia can occur simultaneously, not only in the context of acute but also long-standing emotional stress. Probably, hypothyroidism and migraine make the patient more susceptible to both of these disorders. (Source: Clinical Case Reports)

Acute myocardial infarction due to spontaneous coronary artery dissection in a young male with systemic lupus erythematosus: A case report
Exp Ther Med. 2024 Apr 9;27(6):246. doi: 10.3892/etm.2024.12534. eCollection 2024 Jun.ABSTRACTSystemic lupus erythematosus (SLE) is an autoimmune disease which typically presents in young women. Patients with SLE exhibit features of accelerated atherosclerosis. Here, the present study reported a rare case of acute myocardial infarction (AMI) in a male patient diagnosed with SLE. A 29-year-old male with no cardiovascular history was diagnosed with AMI and underwent coronary angiography, which showed a long-extended spiral-shaped dissection of the right coronary artery (RCA). The patient's autoimmune panel tested positive fo...

Evaluation of the efficacy of silymarin and dexmedetomidine on kidney and lung tissue in the treatment of sepsis in rats with cecal perforation
Exp Ther Med. 2024 Apr 3;27(6):242. doi: 10.3892/etm.2024.12530. eCollection 2024 Jun.ABSTRACTSepsis is a systemic inflammatory response syndrome that develops in the host against microorganisms. This response develops away from the primary infection site and results in end-organ damage. The present study aimed to investigate the protective and therapeutic effects on lung and kidney tissue of silymarin (S) and dexmedetomidine (DEX) applied 1 h before and after sepsis induced by the cecal ligation and puncture (CLP) method in rats. A total of 62 rats was randomly divided into eight groups: i) Control (n=6); ii) cecal perfor...
