Advice for PGY1 seeking GI fellowship...
Hello everyone, I am a recent US-IMG graduate who matched into an "academic" community hospital for Internal Medicine in NYC and will be starting this July. I have a strong desire to get GI fellowship but my program does NOT have an in-house fellowship nor is it very research oriented. I wanted to get an idea from you beautiful people how I can go about securing research and good LORs for GI fellowship application. I have already sent some emails to local GI program directors in the area... Read more (Source: Student Doctor Network)

GI hospitalist
Does anyone have experience to share working as a GI hospitalist? Just doing GI consults for a medium sized hospital and inpatient procedures (general GI), 7 days on 7 off with a PA. Is this something where one would get burned out after a year or two? Average salary experience for this type of job? (Source: Student Doctor Network)

Community Practice Daily Scope Volume
Wanted to see if anyone has anecdotal data on what the daily scope volume looks like from a single endo suite perspective in community practice. I’m thinking 16 scopes daily per suite seems reasonable? Not sure if this is high or low, but thinking 30 min per procedure, operational 10 hrs a day but effective only 8hrs a day when considering turnover time balanced against the time savings when you have an upper/lower and complete in <45 min. Perhaps I am being overly optimistic and it’s... Read more (Source: Student Doctor Network)

Community Hospital Chances for GI?
Hey everyone I’m an incoming intern at a community hospital in the Midwest that doesn’t have an in-house GI fellowship. I had to SOAP unfortunately after interviewing with a couple of academic places but luckily I got into IM program nearby. I’m an AMG and I scored lows 240s on both my step 1 and 2. I’m in the process of working on a GI project with a peds GI in my hometown but other than that I haven’t done much research in the field. I plan to connect with GIs in a nearby university for... pre-gastric bypass egds
average patient in my city is around bmi of mid 30s. Patients coming in for pre-gastric bypass egds routinely encountered have BMIs in 50s and 60s. I think this may be the most dangerous elective procedure we do. No glidescope or anesthesia machine in room. No syringe or alaris pump, manual propofol pushes. Done in a small endo suite far away from the ORs. Despite preoxygenating and running them lighter, using POM NRB, rapid desats are common. Was told by a partner that there was a bad... Preoperative considerations of new long-acting glucagon-like peptide-1 receptor agonists in diabetes mellitus
I've seen something about these drugs maybe replacing metformin as firstline treatment for DM2. Seems like this may become very common for us to see, and this is a nice review of the current concerns we may have: DEFINE_ME Full stomachs: "Nausea and vomiting are explained by direct central effects of GLP-1 and delayed gastric emptying. Both effects decrease over time with ongoing treatment because of tolerance and... Read more (Source: Student Doctor Network)

What operations requires strict NPO?
I see surgeons order in NPO strict or NPO with meds and ice before surgeries, but there doesn’t seem to any obvious reason why they pick between the two. Some surgeons order NPO strict for laparoscopic cholecystectomy and others will be okay with ice and meds for the same procedure. I’m sure there are surgeries and risk factors that require strict NPO (bariatric, gastroparesis, etc), but is there any resources I can review to determine how this is figured out? Thanks in advance! Advanced Endoscopy Application - CV?
Apologies if there's been a thread about this recently. I took a cursory look through last year's advanced endoscopy application thread and it's mostly a list of invites. I'm currently a 2nd-year fellow at a large academic program with an in-house advanced endoscopy fellowship. As a first-year fellow, I was fairly certain I wanted to do general but very recently rotated on the advanced endoscopy service and loved it enough that I'm now kicking myself for not exploring the possibility of... OU-HCOM (Cleveland) vs. NEOMED
Hello, I need help choosing between these two schools and before you instantly write the MD program (NEOMED), I would like you to consider OU-HCOM’s Cleveland campus pros because they are unique for a DO program. I am currently considering pediatric cardiology or pediatric gastroenterology as my most likely choices after medical school. Both have a 3 year IM(pediatric) residency followed by a 3 year fellowship. OU-HCOM (Cleveland) Pros: Partnership with the Cleveland Clinic (second... Read more (Source: Student Doctor Network)
