DIC in septic shock
I have seen a couple of patients recently with profound septic shock ; one from meningococcemia and one from E. coli sepsis, develop DIC and then lose all 4 extremities. I was thinking what would be options for combating the microvascular thrombosis ? Is gastroenteritis that difficult to diagnose clinically....or does EVERYONE need a CT abd/pel?
seriously, as a rad, it's killing me. The list is evergrowing and having to read these essentially negative ct abd/pel on call kills me. Or am I just being dramatic and needs to STFU and just do my job? or dont get me started on these CTA chest/CT abd/pel combos that gets ordered. Sweet baby jesus. We have a doc in our group that has “called out” for things like a gastroenteritis and pneumonia. As horrible as it is I have found it annoying at times because it puts everyone under a lot of stress to cover her. It seems to be an unspoken rule in my group that you don’t just “call out” because you’re sick. “In the hospital” type of sick is different (although one of my coworkers did work through appendicitis and then admitted himself after his shift, but I digress). Appropriate Use of Sick Days
Hi everyone, Just a few days before match day (I matched in Neurology) and I get shaking chills, fatigue and muscle aches. I'm pretty sure this is the flu, despite getting my flu vaccine! It has me thinking, what sort of symptoms or illnesses would constitute appropriate use of sick days in residency? I don't plan on using them for a runny nose, but what about severe gastroenteritis (vomiting every few hours)? Symptoms like I have now? Or should it be limited to things that require ER visit... How thoroughly should you review answers and explanations when doing Qbank questions (usmlerx, uworld, kaplan, and etc)? Its taking me a long time to review questions because im trying to go over each answer choices, find them on first aid, review related topic on it, and do that for each question. I want to open a discussion about a case I feel one of the challenging ones I've seen. 81 year old patient came 2 days ago to the ER with renal colic and a hydronephrosis on the right side and urosepsis was diagnosed. Pre-existing ilnesses: chr. renal failure stage 4, chr. heart failure NYHA 2, multiple heart valve insufficiency.
