Technical Considerations in Laparoscopic-Assisted Endoscopic Retrograde Cholangiography
Am Surg. 2024 Apr 8:31348241244651. doi: 10.1177/00031348241244651. Online ahead of print.ABSTRACTThe use of endoscopic retrograde cholangiography (ERCP) for diagnostic and therapeutic interventions on the pancreaticobiliary system has steadily increased, but the standard approach through the oropharynx is prohibited after Roux-en-Y (RYGB) gastric bypass surgery. Laparoscopic access to the gastric remnant allows for the completion of ERCP using the standard side-viewing duodenoscope to facilitate the completion of standard and advanced endoscopic maneuvers. Here, we describe our experience with the technical aspects of saf...

Weight loss in adult male Wistar rats by Roux-en-Y gastric bypass is primarily explained by caloric intake reduction and pre-surgery body weight
Am J Physiol Regul Integr Comp Physiol. 2024 Apr 8. doi: 10.1152/ajpregu.00169.2023. Online ahead of print.ABSTRACTDiets varying in macronutrient composition, energy density, and/or palatability may cause differences in outcome of bariatric surgery. In the present study, rats feeding a healthy low fat (LF) diet or an obesogenic high fat/sucrose diet (HF/S) were either subjected to Roux-en-Y gastric bypass surgery (RYGB) or sham surgery, and weight loss trajectories and various energy balance parameters were assessed. Before RYGB, rats eating a HF/S (n=14) diet increased body weight relative to rats eating a LF diet (n=20; ...

Conversion of Sleeve Gastrectomy to Roux-en-Y Gastric Bypass: Impact on Reflux and Weight Loss
Sleeve gastrectomy (SG) is the most commonly performed weight loss operation and its two most common complications are postoperative reflux and weight recurrence. There is limited evidence to guide decision-making in treating these conditions. (Source: Surgery for Obesity and Related Diseases)

Preoperative comorbidities as a predictor of EBWL after bariatric surgery: a retrospective cohort study
ConclusionA higher baseline BMI, diabetes mellitus, hypothyroidism, and undergoing laparoscopic sleeve gastrectomy may lead to lower %EBWL in the postoperative period after bariatric surgery. At the same time, patients ’ mental health status and sleep apnea status were not related to %EBWL. This study provides new insight into which comorbidities may need tighter control in order to optimize weight loss outcomes after bariatric surgery.Graphical abstract (Source: Surgical Endoscopy)

Does routine upper gastrointestinal swallow study after metabolic and bariatric surgery lead to earlier diagnosis of leak?
CONCLUSIONS: Routine SS was not associated with earlier diagnosis of leaks compared to the absence of routine SS. Surgeons should consider abandoning the practice of routine SS for the purpose of obtaining earlier diagnosis of postoperative leaks.PMID:38570283 | DOI:10.1016/j.soard.2024.02.005 (Source: Surgery for Obesity and Related Diseases : official journal of the American Society for Bariatric Surgery)

Metabolic Surgery for Diabetes Management
In this report, the authors noted that gastric bypass surgery provided exceptional glycemic control for patients with non–insulin dependent type 2 diabetes. Following these foundational observations, there have been multiple prospectiv e trials that have clearly delineated the superior effectiveness of metabolic surgery for glycemic outcomes in patients with type 2 diabetes and overweight or obesity. For instance, the Surgical Treatment and Medications Potentially Eradicate Diabetes Efficiently (STAMPEDE) trial randomized patients with type 2 diabetes to intensive medical therapy or intensive medical therapy with metabol...

Approaches to Pancreaticobiliary Endoscopy in Roux-en-Y Gastric Bypass Anatomy
The increasing prevalence of bariatric surgery, particularly Roux-en-Y gastric bypass, has necessitated innovative approaches for endoscopic retrograde cholangiopancreatography (ERCP) due to the altered anatomy. Laparoscopy-assisted ERCP offers high success rates but leads to extended hospital stays and an increased risk of adverse events. Enteroscopy-assisted ERCP encounters technical challenges, resulting in lower success rates. A novel technique, endoscopic ultrasound-directed transgastric ERCP, employs a lumen-apposing metal stent to create a fistula connecting the gastric pouch to the excluded stomach, enabling ERCP a...
