Rescue Aortoiliac  Subintimal Recanalization and Stent “Endobypass” for Leriche Syndrome Complicated by Fournier Gangrene
A 67-year-old woman with longstanding Rutherford 4 leg and buttock claudication secondary to aortobiiliac occlusion (Leriche syndrome) presented with Fournier gangrene. Emergent life-saving debridement was complicated by a large nonhealing wound requiring 7 surgeries over 6 weeks. Muscle flap and skin graft surgeries failed because of poor perfusion owing to calcified aortobiiliac chronic total occlusion (CTO) (Fig 1a). The MicroStent HEAL Registry: Interim 6-Month Outcomes
One third of peripheral arterial disease (PAD) patients will advance to critical limb ischemia (CLI)/ chronic limb-threatening ischemia (CLTI), and end-stage disease progression, characterized by rest pain, tissue loss, gangrene, and amputation. Despite ameliorative progress, percutaneous transluminal angioplasty clinical outcomes remain suboptimal. There is still a lack of solutions to improve patency. The HEAL registry collects outcomes data for the treatment of predominantly CLI/CLTI patients implanted with the MicroStent, designed specifically for below-the-knee (BTK) intervention. Meleney's gangrene managed with a single extensive debridement and resultant defect closure with abdominoplasty technique - a case report
CONCLUSION: This case is presented to show how early intervention and radical debridement can improve the outcome in cases of Meleney's gangrene, which is rare and clinically significant. The most common foot complications were amputations, gangrene, foot ulcers, bone deformities, ingrown toenails, Charcot's foot, and calluses. Most DMF patients were anemic and hypertensive. Role of SFGSI, microbial culture and qSOFA as predictive factors in determining the survival rate in Fournier Gangrene patient
CONCLUSION: The survival rate of FG patients can be predicted using the sensitivity and specificity of the SFGSI and qSOFA scores together. P.
