Ganglion of impar RFA
Have a patient that would be a good candidate, 100% pain relief for a month on multiple occasional, coccyx pain and some symptoms with bowel movements. Anyone have experience? Essentially I see people place an RFA needle just like an injection and proceed to sensory testing and ablate. Conventional RF or pulsed? Any rectal injuries? One needle or multiple? Stellate Ganglion Block (cervical sympathetic chain block) with ultrasound guidance; in-plane or out of plane?
Assuming one starts at C6 or C7, Seems to me using an out of plane approach makes a lot of sense, for the following reasons: 1. easier to avoid vessels 2. If standing at head, and needle trajectory is from cranial to caudal trajectory, could push meds further caudal to stellate ganglion. thoughts and preferences? (Source: Student Doctor Network)

PTSD and Stellate Ganglion Blocks
Hello: New to this forum. I am a board certified Pain Medicine MD. I have been seeing patients that are interested in stellate ganglion blocks (cervical sympathetic chain) for PTSD, depression, anxiety, etc. Most of these patients found my practice due to the specificity/complexity of this procedure. Apparently, a '60minutes' CBS piece and a Podcast has sparked interest in this procedure for the aforementioned conditions. I have done my due diligence and cannot find literature... Coccyx pain
I have a patient with several months of coccyx pain. Worse with sitting, TTP over coccyx. Otherwise normal exam. No trauma, but she has an anteriorly displaced coccyx. Failed NSAIDs and no one around me does good pelvic floor PT. I scheduled her for ganglion impar injection, but her insurance won't pay for it. They wouldn't even let me to a peer-to-peer... they just straight up said they won't cover it. Any other treatment options? I don't want to send her to a surgeon. Thanks. 60 mins: SGB for PTSD
Anyone else have experience with these? I've done 3-5 patient for a psychiatrist friend. 2 were clear home runs. The others were lost to follow-up (I just do straight to injection referral). Cash pay. The two who benefited, it was a very rewarding experience. Could a simple shot be a breakthrough treatment for PTSD? Tremor in Parkinson
So the way Kaplan explains Parkinson dz, is that this is the dz in direct basal ganglion pathway. Lack of DA --> suppressed direct pathway --> suppressed motor cortex --> rigidity Which all makes sense. However, how do you explain tremor? Isn't this counter intuitive if Dopamine plays a role in facilitying direct pathway which facilitates movement, such as tremor?
