mRNA Expression Levels of Cytochrome P450 CYP1A2, CYP3A4, and CYP3A5 in the Epidermis: A Focus on Individual Differences Among Japanese Individuals
Xenobiotica. 2024 Apr 22:1-28. doi: 10.1080/00498254.2024.2344664. Online ahead of print.ABSTRACTVarious cytochrome P450 enzymes (CYPs) that contribute to drug metabolism are expressed in the skin. However, variation among individuals in CYP expression profiles is not well-understood.To investigate CYPs related to the metabolism of transdermal preparations in Japan, multiple skin tissue specimens of individuals of Japanese descent were prepared, and the mRNA expression levels of CYP1A2, CYP3A4, and CYP3A5 were measured. Associations between the expression patterns of these CYPs and body mass index (BMI) were also investiga...

A Randomised Controlled Trial Comparing Ketamine < em > versus < /em > Fentanyl for Procedural Sedation in the Emergency Department for Adults with Isolated Extremity Injury
CONCLUSION: PSA is a safe and efficacious procedure for patients undergoing painful procedures in ED. Patients in both the groups maintained hemodynamic stability throughout the procedure. From our study, we were able to conclude that both ketamine and fentanyl are similar in efficacy for PSA in the ED for adults with isolated limb injuries. In addition, no significant cardiovascular adverse events were noted in either group in our study.PMID:38638669 | PMC:PMC11023337 | DOI:10.5704/MOJ.2403.015 (Source: Pain Physician)

A Randomized Controlled Trial of Intrathecal versus Caudal Morphine-Bupivacaine on Postoperative Analgesia and Cortisol Levels in Pediatric Patients
CONCLUSIONS: Intrathecal morphine and bupivacaine results in longer duration of analgesia, lower analgesic consumption, prevents surgical-stress-related elevation of serum cortisol, and improves parent satisfaction compared to caudal morphine with bupivacaine in children undergoing lower abdominal surgeries.PMID:38639890 | DOI:10.1213/ANE.0000000000006962 (Source: Anesthesia and Analgesia)

Transcutaneous electrical nerve stimulation for reducing postoperative acute pain after hip fracture surgery: a double-blinded randomized clinical trial
CONCLUSIONS: The findings of this study indicate that TENS application to the surgical wound, lumbar plexus, and sciatic nerve using anatomical landmarks can significantly reduce postoperative pain, as demonstrated by the higher VAS scores, higher total analgesic consumption and high additional analgesic need in the control group after hip fracture surgery.CLINICALTRIALS: gov ID: NCT02642796.PMID:38639518 | DOI:10.26355/eurrev_202404_35907 (Source: Pharmacological Reviews)

When Buprenorphine is Not Enough: A Case of Transitioning Off of Buprenorphine in Cancer Pain and Opioid Use Disorder
1. Participants will familiarize with the available evidence for different opioid rotation strategies when converting between buprenorphine and other opioids.2. Using a case-based approach, participants will review and self-report the ability to use a conservative strategy for rotation from buprenorphine sublingual to transdermal fentanyl. Evaluation of Postoperative Pain After Cardiothoracic Surgery in Patients With and Without Significant Preoperative Anxiety: A Prospective Observational Study
CONCLUSIONS: This clinical trial demonstrated that greater than 60% of the participants presented with preoperative anxiety, the majority being young participants. Lower socioeconomic status is also a risk factor for preoperative anxiety. Patients who suffered from preoperative anxiety are more likely to have greater pain scores and analgesic needs during postsurgical assessment.PMID:38607875 | DOI:10.4103/aca.aca_175_23 (Source: Annals of Cardiac Anaesthesia)

Effect of an intravenous acetaminophen/ibuprofen fixed-dose combination on postoperative opioid consumption and pain after video-assisted thoracic surgery: a double-blind randomized controlled trial
ConclusionPerioperative administration of acetaminophen/ibuprofen significantly decreased analgesic needs in patients undergoing VATS, providing an effective postoperative pain management strategy, and potentially minimizing the need for stronger analgesics. (Source: Surgical Endoscopy)
