What do you think of this ECG that was texted to me?
This ECG was texted to me by a colleague.  I assumed it was from a patient with symptoms compatible with ACS/OMI.What do you think? There is sinus rhythm with RBBB and ST Elevation in V2 which is concordant with the QRS.  The ST segment in RBBB should be ST depression, discordant to the positive R ' -wave (i.e., opposite direction of the upright R ' -wave).  But we sometimes see this concordant STE even in non-ischemic ECGs:PseudoSTEMI and True ST elevation in Right Bundle Branch Block (RBBB). Don ' t miss case 4 at the bottom. This STE suggests OMI, but we have seen many cases like this th...

Eko ’s Newest CORE 500 Stethoscope: A Review
Arriving in two boxes reminiscent of Apple product packaging – one for the chest piece (the part that contacts the body), and another for the detachable earpiece (tubes + ear tips) – the CORE 500 is clearly an upgrade from the Eko DUO stethoscope. Similar to its predecessor, the CORE 500 can be used with your own headphones, smart phone, and just the chest piece, making it free of any hassle with the earpiece tubes. It has a USB-C charging port and is good for a continuous five-hours of use. The CORE 500 chest piece itself is a bit heavier than the DUO at 6.6 oz (instead of 3.6 oz), but the construction feels a lot ...

We got the new COVID vaccine. It was a whopping $191 per shot.
As emergency physicians, my husband and I were determined to protect ourselves, our teenagers, and society at-large with the latest, monovalent COVID vaccine, which the CDC recently recommended for every American 6 months and older. I realized, though, we were in for turbulence when my 78-year-old mother arrived for her appointment at a national pharmacy chain Read more… We got the new COVID vaccine. Tomb of the unknown emergency physician
An overly familiar alarm buzzes at 4:20 a.m. My first thought was, “I should have slept more.” I lazily brush my teeth with the combination of spearmint and reflux mixing in the back of my throat. With heavy arms and legs, I wearily climb into scrubs in the dark and carefully exit the house – Read more… Tomb of the unknown emergency physician originally appeared in KevinMD.com. Metric shaming in medicine and 3 ways to overcome it
Looking back at my career as an emergency physician, I remember how my first year out of residency was a roller coaster ride with many ups and downs, practicing as an emergency physician in a semi-rural town in Illinois. But once I found my stride and confidence as an attending physician, I recall distinctly knowing Read more… Metric shaming in medicine and 3 ways to overcome it originally appeared in KevinMD.com. A man in his 60s with syncope and ST depression. What does the ECG mean?
Written by Sean Trostel MD, peer reviewed by Meyers, Smith, Grauer, etc.A man in his 60s with a history of severe alcohol use disorder and epidural abscess on long-term ciprofloxacin presented to the emergency department after an episode of syncope while standing in line at a grocery store. He did not have chest pain. Here is his triage ECG:   What do you think? My interpretation:Sinus rhythm, normal QRS, widespread wavy ST depression leading into late T/U-waves with very prolonged QT. No evidence of OMI. QTc/QUc is in the range of 630 msec. What is the most likely cause of the patient ’s ECG findings, and wha...

Turning adversity into hope: my path to a career in medicine
I have wanted to work in medicine ever since my eleven-year-old cousin passed out during my uncle’s birthday party. He suddenly fainted when we were supposed to cut the cake. We rushed to the emergency room. Doctors diagnosed him with blood cancer after a couple of months, which was too late for him to survive. Aspects of Social Determinants of Health: Specific Types of Help
This is the final article in a series about social determinants of health (SDoH). Previous articles covered the definition, collection, sharing, and use of SDoH. I’ll finish with a look at some specific areas of intervention. We’ve seen the importance of personal interventions, through interviews and evaluating SDoH, to solve problems. In rural areas, where it might be daunting either to travel to a doctor or to schedule an online visit with poor Internet access, home visits fill the gap. Conduit Health Partners is one organization that does so. A patient can speak to a nurse immediately—as opposed to calling...

“ Low quality ” Interventions in “ high quality ” cath labs
Recently, I visited a seven-star cardiology center. It had a fascinating lab with a near-360-degree roaming arm and floor (even the roof, I think!). The lab also had provision for optional thoracotomy support by surgeons and a CT scan. What’s more, I was told that an adorable robot will soon be added to the cath lab workforce to assist in delivering and maneuvering catheters. It’s truly amazing to see the advancements in science! I asked the chief staff nurse how many procedures are being performed per day. She pulled out an Excel chart from a gesture-controlled touch panel. It was clocking about 20 to 25 ca...
