An Unusual Cause of Laryngotracheobronchitis
We present a case of pseudomembranous cryptococcal laryngotracheobronchitis in an 80-year-old woman with a history of intractable cough with production of long sputum strings and severe dysphonia. Her main risk factor was considered to be the use of high-dose inhaled fluticasone, given for severe mixed asthma/chronic obstructive pulmonary disease (COPD). This is an unusual presentation of pulmonary cryptococcal disease, which commonly presents as parenchymal disease. Authors of the Guidelines for the Prevention and Treatment of Opportunistic Infections in HIV-Infected Adults and Adolescents have recently released updates to the following sections:
HPV - This update provides new recommendations regarding the cervical cancer screening interval for HIV-positive women after 3 consecutive normal Pap tests, and the use of HPV co-testing among HIV-positive women >30 years of age. Some HIV-infected patients with certain gene at increased risk for fungal disease
HIV-infected people who carry a gene for a specific protein face a 20-fold greater risk of contracting cryptococcal disease, according to a study published in mBio®, the online open-access journal of the American Society for Microbiology. Cryptococcosis and Meningitis
"Cryptococcosis," is an infection that is caused when a person inhales the fungus, "Cryptococcus Neoformans." The disease most often affects people with AIDS. While it might be limited to a person's lungs, it frequently spreads throughout a person's body. Although nearly any organ in the body can become infected, the fungus is often times fatal if it infects a person's nervous system where it causes an inflammation of the membranes covering the spinal cord and brain - referred to as, "meningitis."
