Rapid Endoscopic Diagnosis of Benign Ulcerative Colorectal Diseases with an Artificial Intelligence Contextual Framework
Benign ulcerative colorectal diseases (UCDs) such as ulcerative colitis (UC), Crohn ’s disease (CD), ischemic colitis (IC) and intestinal tuberculosis (ITB) share similar phenotypes with different etiologies and treatment strategies. To accurately diagnose closely-related diseases like UCDs, we hypothesize that contextual learning is critical in enhancing the ability of the artif icial intelligence models to differentiate the subtle differences in lesions amidst the vastly divergent spatial contexts. Myeloid Cells and Sphingosine-1-Phosphate Are Required for TCR αβ Intraepithelial Lymphocyte Recruitment to the Colon Epithelium
J Immunol. 2024 Apr 3:ji2200556. doi: 10.4049/jimmunol.2200556. Online ahead of print.ABSTRACTIntraepithelial lymphocytes (IELs) are T cells important for the maintenance of barrier integrity in the intestine. Colon IELs are significantly reduced in both MyD88-deficient mice and those lacking an intact microbiota, suggesting that MyD88-mediated detection of bacterial products is important for the recruitment and/or retention of these cells. Here, using conditionally deficient MyD88 mice, we show that myeloid cells are the key mediators of TCRαβ+ IEL recruitment to the colon. Upon exposure to luminal bacteria, myeloid cel...

Gut metabolome and microbiota signatures predict response to treatment with exclusive enteral nutrition in a prospective study in children with active Crohn's disease
CONCLUSIONS: We identify microbial signals and diet-related metabolites in feces, which could comprise targets for pretreatment optimization and personalized nutritional therapy in pediatric CD.PMID:38569785 | DOI:10.1016/j.ajcnut.2023.12.027 (Source: The American Journal of Clinical Nutrition)

Parenteral n-3 polyunsaturated fatty acids supplementation improves postoperative recovery for patients with Crohn's disease after bowel resection: a randomized, unblinded controlled clinical trial
CONCLUSIONS: Parenteral n-3 polyunsaturated fatty acids supplementation promotes postoperative recovery in patients with Crohn's disease following bowel resection, with fewer complications and reduced inflammatory cytokines. This trial was registered at clinicaltrials.gov as NCT03901937 at https://classic.CLINICALTRIALS: gov/ct2/show/NCT03901937?term=NCT03901937&cond=Crohn+Disease&draw=2&rank=1.PMID:38569774 | DOI:10.1016/j.ajcnut.2023.12.022 (Source: The American Journal of Clinical Nutrition)

Beyond diagnosis: investigating hospital referral impact on biological treatment initiation, hospital admission, and surgery patterns in inflammatory bowel disease - a Danish population based study
CONCLUSIONS: Based on the findings of this population-based study, we found no evidence that the referral area had a significant impact on the duration from diagnosis to the initiation of biological treatment, hospital admissions, or surgery. However, the data suggested that fewer patients with luminal Crohn's disease were referred to biologics from Non-Academic Hospitals.PMID:38567864 | DOI:10.1080/00365521.2024.2337829 (Source: Scandinavian Journal of Gastroenterology)

Effectiveness and safety of adalimumab biosimilar in patients with inflammatory bowel disease
CONCLUSIONS: Between originator adalimumab and biosimilar-start cohorts, no differences were observed, between originator adalimumab and switch cohorts, no significant differences were found either, and with the pre- and post-switch to biosimilar comparison, 2 of the 9 patients experienced AEs after the switch. The biosimilar showed a favorable safety profile (one patient with a serious adverse effect (rash) with biosimilar discontinued treatment) and no significant changes to clinical or biochemical parameters were observed after the switch.PMID:38570210 | DOI:10.1016/j.farma.2024.01.002 (Source: Farmacia Hospitalaria)
