Non-vitamin K oral anticoagulants versus warfarin for the treatment of left ventricular thrombus
CONCLUSION: Non-vitamin K oral anticoagulants may be an efficacious and safe alternative to warfarin for LVT management. Future studies should explore the safety and efficacy of anticoagulation discontinuation by 1 year as an overall strategy.PMID:38327164 | DOI:10.12809/hkmj2210034 (Source: Hong Kong Med J)

Acute Kidney Injury in Different Anticoagulation Strategies: A Large-Scale Pharmacoepidemiologic Study Using Real-World Data
ConclusionParticular attention should be directed toward monitoring renal function in individuals receiving anticoagulants, including warfarin and direct oral anticoagulants, as well as other anticoagulant agents. This diligence is particularly imperative within the first month after anticoagulant administration for individuals with a tendency for AKI. (Source: Cardiovascular Drugs and Therapy)

Trends in Deep Vein Thrombosis Prophylaxis After Total Knee Arthroplasty: 2016 to 2021
In 2011, the American Academy of Orthopaedic Surgeons released a consensus recommending venous thromboembolism (VTE) prophylaxis after total knee arthroplasty (TKA). The purpose of our study was to examine: (1) incidences of postoperative complications, including pulmonary embolism (PE), deep vein thrombosis (DVT), and transfusion rates; (2) trends from 2016 to 2021 in VTE prophylaxis; as well as (3) independent risk factors for 90-days total complications following TKA between aspirin, enoxaparin, rivaroxaban, and warfarin. Facility-Level Variation in Racial Disparities in Anticoagulation for Atrial Fibrillation: The REACH-AF Study
ConclusionsIn a national cohort of patients with AF, we observed large facility-level variation and adjusted risk differences in any anticoagulant and DOAC initiation, overall and by race. These findings represent a target for local quality improvement in AF care. (Source: Journal of General Internal Medicine)

Coil Embolization of a Large Recurrent Pulmonary Arteriovenous Malformation by Retrograde Transseptal Pulmonary Vein Access
A 55-year-old woman presented with stroke and exercise intolerance and was found to have a complex right lower lobe pulmonary arteriovenous malformation (PAVM). The PAVM morphology was complex with an extensive number of small feeding arteries/arterioles and 1 dominant draining vein (Fig 1). The PAVM was treated with transarterial embolization in 3 separate procedures over a 6-year time span, using multiple vascular plugs and coils. The patient was also diagnosed with antiphospholipid syndrome and started on warfarin anticoagulation. (Source: Journal of Vascular and Interventional Radiology : JVIR)

Risk scores for major bleeding from direct oral anticoagulants: comparing predictive performance in patients with atrial fibrillation
CONCLUSION: ORBIT-AF and ATRIA provided the best bleeding risk prediction within the first 1100 days. None of the 6 bleeding risk scores provided predictive benefit over 2100 days of DOAC treatment.PMID:38292348 | PMC:PMC10826825 | DOI:10.1016/j.rpth.2023.102285 (Source: Thrombosis and Haemostasis)

Modified application of SAMe-TT2R2 scoring system in Asian patients with atrial fibrillation for the selection of oral anticoagulants
CONCLUSIONS: The modified SA2Me-TTR score was useful for guiding oral anticoagulants selection in Asian patients with AF.PMID:38287499 | DOI:10.3904/kjim.2023.381 (Source: The Korean Journal of Internal Medicine)

Direct oral anticoagulants versus vitamin K antagonists in patients with atrial fibrillation and stage 5 chronic kidney disease under dialysis: A systematic review and meta-analysis of randomized controlled trials
ConclusionThis meta-analysis suggests that in patients with AF on dialysis, the use of DOACs was associated with a significant reduction in stroke, and a numerical trend of less incidence of major bleeding compared with VKAs, but in this case with no statistical significance. Results may be limited by a small sample size or insufficient statistical power. (Source: Journal of Thrombosis and Thrombolysis)

Transition of management strategies and long-term outcomes in cancer-associated venous thromboembolism from the warfarin era to the direct oral anticoagulant era
CONCLUSIONS: In the current historical comparison of cancer-associated VTE between the 2 large real-world registries, there was a striking change in the treatment strategies with decreased risks of recurrent VTE and major bleeding in the DOAC era compared with those in the warfarin era, while there seemed to be unmet needs of DOAC-related gastrointestinal bleeding.CLINICAL TRIAL REGISTRATION: URL: http://www.umin.ac.jp/ctr/index.htm UNIQUE IDENTIFIER: UMIN000044816.PMID:38278660 | DOI:10.1016/j.ejim.2024.01.012 (Source: European Journal of Internal Medicine)

Venous thromboembolism, chronic liver disease and anticoagulant choice: effectiveness and safety of direct oral anticoagulants versus warfarin
CONCLUSION: While the magnitude of clinical benefit varied across individual DOACs, in adults with acute VTE and chronic liver disease, oral factor Xa inhibitors (as a class or individually) were associated with lower risk of recurrent VTE and major bleeding.PMID:38268519 | PMC:PMC10805675 | DOI:10.1016/j.rpth.2023.102293 (Source: Thrombosis and Haemostasis)

Prevalence of Guideline Discordant Aspirin use and Associated Adverse Events in Patients on Warfarin for Mechanical Valve Replacement
Antithrombotic therapy guidelines change over time due to evolving research and available treatments. For concomitant aspirin in patients on vitamin K antagonists for ischemic stroke prevention in mechanical heart valves, guidelines have shifted. In 2020, citing the fact that previous studies included many patients with older-generation valve prostheses and acknowledging the increased risk of major bleeding, the American College of Cardiology (ACC) and American Heart Association (AHA) reversed their previous recommendations to routinely add aspirin to VKA therapy.
