Out of curiosity: Let's say you make a certain score on USMLE that exceeds the filter score for a given program, but do not exceed that filter score on COMLEX. Would you still make it past the filter for that program? Cough. My COMLEX is bad :>. (Source: Student Doctor Network)

Interesting case
Thought I’d post an interesting case I’ve had recently and hear some opinions as to how people would have approached it. Will post the case sequentially. Some details changed to protect privacy. 30 yr old male had a MVR 10 years ago for infective endocarditis. Other medical hx is recent onset seizures for which he was started on Phenytoin 1 month ago. Promethazine/codeine on top of Percocet and diazepam?

Can we start a COVID-19 discussion? There is one case at my hospital so far. He got sick initially with a prodrome of cough, malaise... presented two weeks later to ED with severe respiratory symptoms. Got admitted, bilateral infiltrates more in the peripheries. WBC normal with lymphopenia. Flu, RSV and general viral panel negative. Tested two days after admission for COVID-19 which came back positive. Community infection with no travel or sick contact. Clinical case management - IIIA NSCLC
This is something that I've realized is extremely regional and doesn't seem to follow anything except what the group has decided they'd like to do. At residency we treated one way, first job treated another way, and now another way. Example case: 64 year old man with 35 pack year history presents with cough and slight worsening of SOB. Malpractice Case: Missed x-ray finding
Case 8: Cough – medmalreviewer Patient seen for cough. Radiology read mentions incidental finding. No follow-up until several years later, patient's incidental finding now developed into inoperable cancer. Patient sues. Doc has died in the meantime, so they're really just suing his estate. Lawsuit dismissed due to statute of limitations. I'm guessing the doc looked at the xray, searching for pneumonia, did not see any, so discharged... ESP block
Seeing more and more in the literature. Several nice benefits. Used it again today: surgeon unexpectedly opened up a lap choley and pt had huge incision just under rib cage and I wasn’t sure if TAPS would do it. 0/10 pain in pacu. Even witnessed him cough without flinching a bit and ask for more ice chips. Other cases I’ve used it in: thoracotomy (lung, lvad, mini mitral), mastectomy, combined open liver + bowel resection Are you guys incorporating this more? Has it gained any... Necessity of Lactate for every person with a potential infection
I've been thinking about this a lot over the past several months. Where I work, if a patient has an infection or potentially an infection, it's almost reflexive to order a lactate. This is irrespective of the patient history, vital signs, or physical exam. For instance, we (meaning the ER docs) get lactates on the following - routine leg cellulitis of a middle aged patient with no PMH and normal vitals signs. - cough with temp 99.6, everything else on H&P is OK - a butt abscess and HR...
