Chronic pain, CKD, etc. Presented at valve conference for failure of mitral bpv. Mean gradient of 15 mmHg. It was decided that we would proceed with VIV TMVR with a Sapien 3. Can I be a CT surgeon and still have a life?
Today, I just finished a sub-I in CT surgery at my medical school in Southern California. It was my favorite rotation so far in medical school. Even with all of the stress and chaos involved, I found myself excited to wake up every morning and scrub into a CABG or valve replacement or an aortic dissection. I felt extremely connected to the various residents, fellows, and attendings I worked with. Some of them even told me I would have a really good chance at getting accepted to my med... Critical Care Nurse taking Postbac Courses? How does this experience look for Admins
Hey guys I just got a RN position on a CTICU unit at the #1 Hospital in New York . This unit gets all the open heart surgeries, CABG, vents, ECHMO, etc. I'm super stoked and super humbled. I was a med-surg nurse for 4 years prior to the switch. For my Nurses turned to Physicians, how much has your nursing experience impacted your pre-med journey? I could imagine it could only help. Current Pre-Med situation : By the time I matriculate (assuming I apply 2023 cycle and accepted for the... Read more (Source: Student Doctor Network)

Cardiac Induction Case
For the students and residents. 70 something y/o pt for CABG. EF 30-35%, pretty bad CAD. ESRD, last HD last night. Inpatient. Pt is tiny, and anemic to boot. Hct high 20s. Looks frail. Edentulous. Has that aura of "I'm gonna give you trouble today." To the room and baseline SBP 180s. Induce gently and the pressures hold in the 100s throughout line placement. Yay, we're out of the woods I thought. I thought. Then all of a sudden the pressure drops to the 60s, and the sats drop to the... Cardiac Induction Case (Source: Student Doctor Network)

Another Cardiac Case
I always enjoy the case presentations around here and it's been a while since I contributed, so here was a fun case 54 y/o male underwent Sternotomy for CABG/MVR/MAZE/LLAA 2 months ago after presenting with an NSTEMI. BMI 42, recent hx of Meth and Marijuana use, current everyday smoker with questionable COPD (PFTs were fine), OSA on BiPAP while at hospital (non-compliant at home), and AFib on Elliquis. Surgery went uneventfully, had a Mitral Valve Repair (simple ring), good flow in... Another Cardiac Case (Source: Student Doctor Network)

Challenging case
Had an interesting case recently, wondering how others would have tackled it: 72 yo M with poorly controlled HTN, IDDM, morbidly obese (190kg), pAF, CAD s/p remote CABG complicated by ischemic CM (LVEF 25% w/ RWMAs), post-capillary pulmonary HTN (mPAP ~50, R heart dilated and mildly reduced fxn), and AS s/p TAVR now w/ severe prosthetic valve stenosis. ESRD on HD, however he is now progressively volume overloaded because he has been getting hypotensive during attempts at ultrafiltration... Challenging case (Source: Student Doctor Network)

Severe pHTN for bilateral debridement @ Surgicenter : (
Hey everyone, Interesting case coming up I thought I’d solicit some ideas on. 57 yo F with history of primary pHTN, COPD, OSA non-compliant with CPAP (nocturnal nasal cannula 2L instead), CABG in 2001, DES 4 mo. ago to SVG-RCA. LIMA-LAD patent, otherwise just luminal irregularities and lesions <40%. Scheduled for bilateral venous stasis ulcer debridement at the surgicenter. No angina. Can “just about” lay flat. But minimal exercise tolerance due to SOB. Sees pHTN specialist who says... Severe pHTN for bilateral debridement @ Surgicenter : ( (Source: Student Doctor Network)

Urology vs ENT.... Or Plastics
Medical Student here. I started medical school thinking i wanted to be a cardiac surgeon after doing a year of research in cardiology prior to medschool. Unfortunately, after watching 20 odd CABG I’ve realised cardiac surgery isn’t for me, I want a speciality that is a little broader. So as an overview I would like to be in a surgical speciality which offers: - Diversity both in patient population and procedures - A medical and diagnostic element - Reconstructive in nature - A bonus would...
