How would you approach this case?
A little similar to thread below but would like you opinions! Learning a lot from these boards even after residency! Alone at night, patient brought in from nursing home,, 75 yo M 70kg ESRD, HTN, A fib on eliquis, DM, CAD LAD stent 10 yrs ago, CHF EF 10% on coreg, mod MR, mod TR , dementia, severe pHTN, reduced RV function w severely elevated CVP, fell, now w slowly expanding thigh hematoma needs to be drained. No detailed info/cath regarding CAD. Vitals normal/stable after 2 unit... How would you approach this case? (Source: Student Doctor Network)

"Hemodynamically Benign" RSI
Hi. IM/CCM here. I thought about posting this in CCM but wanted an anesthesiology opinion (or a few): called in the middle of the night to see a (strong/hearty) 82 y/o man with 1.5 liters of coffee ground liquid in his newly placed NG tube. HR 70s (but on Coreg), 80s/50s, mildly tachypneic, 7.34/48/62/17 with a lactate of 8. Bolus 1 liter of LR and 2 units prbc with BP to 122/70. Can we prevent chemo related cardiac damage?
I think this study is very exciting- Enalapril and carvedilol for preventing chemotherapy-induced left ventricular systolic dysfunction in patients with malignant hemopathies: The OVERCOME trial. J Am Coll Cardiol 2013; DOI: 10.1016/j.jacc.2013.02.072. Available here. In patients with haematological maligancies pretreatment with enalapril and carvedilol appears to ameliorate the reduction in LV ejection fractionBUT it is only a small study with a small change in EF so I guess we need  a larger study but certainly appears promising!sadian (Source: Doc2Doc BMJ Cardiology)
